2. Item V.19. —There are setback parameters within 500 feet of the treatment/storage units with field
observed distances not listed in the table (i.e., proposed on -site habitable residence, proposed well,

c. How is the UV dosing rate of 25 GPM higher than the provided unit flow of 10 GPM?

1. There is a duplicate cover sheet and table of contents for the UV Installation and Maintenance
Guide. One cover sheet says "Maintenance only. Installation is in Equipment Section" and the other
says "Installation Only. Maintenance is in O&M Section". There is no maintenance guide included
for the UV unit in the Operations & Maintenance Plan. Please add this.
