In accordance with your request to provide an intermediate 0.006 MGD limitation page in your
NPDES permit to accommodate your December 12, 2014 Authorization to Construct application for
the first phase of construction for 0.006 MGD treatment capacity, a minor permit modification is being
issued. As we discussed, except for the permitted flow both limitation pages, 0.006 MGD and 0.012
MGD have the identical limits and monitoring requirements. To expand to 0.012 MGD will require
you to obtain an Authorization to Construct permit for the 0.012 MGD expanded flow. Additional
technical corrections were made to narratives and formatting where warranted.
Please insert the attached minor modification pages in your existing permit and discard any
pages that are duplicated by the minor modification pages.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This permit does not affect the
legal requirements to obtain other permits which may be required by the Division of Water Resources
or any other Federal, State, or Local governmental permits that may be required.

ADDENDUM — FACT SHEET NC0089478 Camp Watia WWTP — May.19 2015
An Authorization to Construct (A to C) application was received 12/12/14 for yet- to- be built domestic
treatment plant for NC0089478 Camp Watia WWTP. The NPDES permit provided a limitation page for
0.012 MGD but the A to C application was for a phase I initial plant operation/design of 0.006 MGD. To
proceed with the A to C permit, a 0.006 MGD limitation page was required in the NPDES permit. After
discussions with the permittee, it was agreed a minor NPDES permit modification would be issued
adding the intermediate 0.006 MGD limitation page with identical limits and monitoring requirements,
except for the flow limit, as stated for the 0.012 MGD limitation page. Because of the addition of the
0.006 MGD flow page, to expand to 0.012 MGD will require the permittee to obtain an A to C permit and
submit a signed Engineer's Certificate for the expansion. Various technical corrections to narratives and
formatting were made where required.
