Fast Track Authorization to Construct Permit No. SWA000028
Nesbitt Farms Subdivision — Phase 1

Dear Mr. Taylor:
The Post -Construction Stormwater Fast Tract Authorization to Construct (ATC) Permit SWA000028 for the
subject project expires in less than 180 days on August 2, 2023. An expiration date reminder letter was
previously sent on December 20, 2022, in advance of this expiration date, advising of your obligations under this
permit as well as the available options to keep this project under an active post construction stormwater permit.
Since a renewal application has not been submitted a minimum of 180 days prior to this expiration date, please
proceed with one of the following options:
Apply for and receive a permit through the standard permitting process by submitting for review a new
application package that includes but is not limited to:
a. a new Permit Application Form (SWU-101)
b. a $505.00 fee
c. a completed Supplement-EZ form
d. a recorded or completed O&M-EZ form
e. as -built plans and calculations for any completed SCMs
f. the full or partial designer certifications for any SCMs or project areas that have been completed
(per Permit Condition 3.e)
g. a completed rescission form to be acted upon once the new permit is approved.

2. If construction has not started and there are no longer plans to construct the project such that a permit is
no longer needed, please submit a permit rescission request form, which can be found on the Post -
Construction section of this website.
North Carolina General Statutes and the Coastal Stormwater rules require sites that have started construction to
remain covered under an active post construction stormwater permit. Failure to maintain a permit subjects the
owner to enforcement action as set forth in NCGS 143 Article 21, including the assessment of civil penalties of
up to $25,000 per day. Any enforcement actions taken by the Division will consider the status of construction as
well as any applications submitted for review.
