Division of Water Resources INSTRUCTIONS FOR FORM CSA 04-16&SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
The copy may be submitted in digital format.
A. Cover Letter
II Submit a cover letter listing all items and attachments included in the permit application package
B. No Application Fee Required
> No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit
> The appropriate annual fee for systemwide wastewater collection system permits may be found at:

C. System-Wide Wastewater Collection System (FORM: CSA 04-16)Application:
Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
❑ If the Applicant Type in Section I.3 is a Privately-Owned Public Utility, provide the Certificate of Public
Convenience and Necessity(CPCN)from the North Carolina Utilities Commission demonstrating the Applicant is
authorized to hold the utility franchise for the area to be served by the wastewater collection system,or
> Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a franchise has been received and that the service area is contiguous to an existing franchised area
or that franchise approval is expected.
❑ If the Applicant Type in Section I.3 is a corporation or company, provide documentation if it is registered for
business with the North Carolina Secretary of State.

accordance with 15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated

PURPOSE:
The purpose of this document is to protect the citizens of Henderson County, North Carolina,
and the environment by establishing written procedures for responding to Sanitary Sewer
Overflows (hereafter referred to as SSO's). This procedure applies to all Water and Sewer
employees that in the course of their work may deal with SSO's. These procedures will be
revised at least annually, or whenever revised guidance from County,State or Federal
authorities dictate a revision is necessary.
A. REPORTING.
1. Individual calls the Field Operations or Customer Service office (Finance Department) or
Administration office (Water and Sewer Department), during normal office hours, or the
Hendersonville Water Treatment Plant, after normal office hours, or on the weekend, or
on a holiday to report a sewer main break, sewer main blockage, damaged sewer line,
or other sewer utility problem.
2. The City personnel receiving the call should obtain the following information
(minimum):
a. Name of Caller.
b. Name of company or public entity, if applicable.

c. Address of Caller.
d. Telephone number (work, cellular, and/or home) of Caller.
e. Reason for call.
f. Location of problem(s), if different from address above.
g. Date and time of call.
h. Name(s) of company and/or person responsible for damage(s), if applicable.
i. Name(s) of company for whom they are contracted by, if applicable.
j. Name(s) of other utilities that may be involved.
3. Upon discovery of a possible SSO site during normal duty hours, notify the SSO Contact
Monitor(s) at the telephone numbers listed on the call out sheet.
4. After normal work hours contact Collection System Supervisor. In the event that he
cannot be reached, contact Distribution System Supervisor. In the event that he cannot
be reached, contact the Facilities Maintenance Supervisor. In the event that he cannot
be reached, contact the Wastewater Treatment Plant Superintendent. In the event that
these individuals cannot be reached, call the Water Treatment Plant at 891-7779, and
have them contact the Operations Manager or Utilities Director.
5. SSO Contact Monitor(s) (refer to Attachment 1—SSO Contact Monitors)will be the
primary interface between the City of Hendersonville and the State of North Carolina
Department of Water Quality on all SSO matters.
6. SSO Contact Monitor(s) will notify the Operations Manager of any SSO that falls within
the parameters below. After normal duty hours he can be reached by cell phone or by
pager (refer to call out sheet). The Operations Manager will be responsible for notifying
the Utilities Director.
7. SSO Contact Monitor(s) will report overflows from any sewer line or pump station or the
bypass of any wastewater treatment system or any component thereof.This
requirement applies in the following cases.
a. Any spill of 1,000 gallons or greater;
b. Any spill, regardless of the volume, that reaches the surface waters of the State.
Please note that drainage ditches and storm sewers are considered waters of the
State;
c. In the event a spill occurs, as described in 7.a. or 7.b., the City will issue a press
release notifying the public of said spill.The City will submit the same press release
to the ARO.
d. SSO Contact Monitors are required to provide an oral report regarding reportable
spills to the appropriate Regional Office as soon as possible, but in no case later than
twenty-four (24) hours following the spill event at (828)296-4500;
e. Additionally, if a spill occurs after normal business hours, the event must be
reported during the next working day. Should a spill occur after hours that reaches

the surface waters and/or it may be considered as an environmental or health
hazard, the report must be made immediately to the North Carolina Division of
Emergency Management (NCDEM) at 1-800-858-0368 and the NCDEM office will
contact appropriate Division of Water Quality staff.
f. A written report (see attached form) must be submitted to the appropriate regional
office with in five (5) days of the time that the City of Hendersonville becomes aware
of the spill occurrence.All parts of the form must be completed in detail and
submitted either at the time of the oral report or within the required five (5) days. It
is highly recommended that the form be emailed to the regional office, followed up
by a telephone call to confirm the fax has been received and to provide any

additional information that may be needed.
B. CAUSE DETERMINATION AND RESPONSE.
1. CAUSE DETERMINATION.
a. Line Blockage; or
b. Damaged Line; or
c. Pump Station Failure.
2. RESPONSE:
a. Line Blockage:
(1) Locate the end of the spill area either in an open ditch or the point where a
storm drain empties. Create a dam or insert a plug to prevent spill from
progressing further.The dam needs to be constructed near a manhole or at a
location that is accessible to the jet/vacuum truck. Build the dam using dirt or
sand bags that are capable of containing water flow from fire hydrant(s)without
dam failure. Have backhoe at site with extra material at all times so dam can be
rebuilt if a breach occurs.
(2) Put up warning tape, black on red or black on yellow "Danger Do Not Enter",
from the SSO site to the pump site on both sides of the open ditch in populated
areas. In unpopulated areas restrict access where there is any chance of humans
coming in contact with the SSO.
(3) Station two (2) pumps at the dam that are capable of maintaining a pumping
rate equal to or greater than fire hydrant flow rate.Use one pump for backup.
There will be two(2) workers at the pump site at all times during pumping
operations. Pump effluent from dam into the manhole and/or vacuum truck.
(4) Locate blockage simultaneous to pump around operations.
(5) Clean line out with jetting machine enough to re-establish adequate flow
through the pipe line while removing blockage material with vacuum truck.

(6) Flush ditch or storm drain with water from a sufficient number of hydrants to
remove waste product from ditch. Notify the water treatment plant when
starting and stopping of ditch flushing.
(7) If rip rap is in ditch,wash down with high water pressure until there is no waste
residue remaining on, or within the rip rap structure.
(8) Continue to pump effluent from dam into the manhole until all water has been
reclaimed.
(9) Once finished cleaning spill, return to blockage point and clean line thoroughly,
while removing blockage material with vacuum truck.
(10) Proceed to clean-up operations (Section C).
b. Damaged Line:
(1) Locate the end of the spill area either in an open ditch or the point where a
storm drain empties. Create a dam or insert a plug to prevent spill from
progressing further.The dam needs to be constructed near a manhole or at a
location that
is accessible to the jet/vacuum truck. Build the dam using dirt or sand bags that
are capable of containing water flow from fire hydrant(s)without dam failure.
Have backhoe at site with extra material at all times so dam can be rebuilt if a
breach occurs.
(2) Put up warning tape, black on red or black on yellow "Danger Do Not Enter",

(4) Locate and expose damaged line simultaneous to pumping operations.
(5) If gravity sewer, plug line upstream and downstream from damaged section at
manhole,whenever possible. Set up one (1) pump in an upstream manhole
capable of maintaining a pumping rate equal to or greater than that flow rate in
the sewer main. Pump from this upstream manhole to a downstream manhole
and/or to the vacuum truck. If the damage line is a pressure sewer, have all
pump stations pumping into line shut down. Ensure availability of a vacuum
truck to pump down pump station wet wells as required.

(6) Continue pumping discharge from line to downstream manhole and/or vacuum
truck until flow has ceased enough to repair or replace damaged section of line.
(7) Flush ditch or storm drain with water from a sufficient water source (i.e., creek
upstream of SSO or a fire hydrant)to remove wastewater from ditch or storm
draining system. Notify water treatment plant personnel prior to initiation of and
upon completion of all flushing operations.
(8) If rip rap is in ditch,wash down with high water pressure until there is no
wastewater residue remaining on, or within the rip rap structure.
(9) Continue to pump effluent from dam into the manhole until all water has been
reclaimed.
(10) Proceed to clean-up procedure (Section C).
c. Pump Station Failure:
(1) Pump down pump station well to terminate SSO and notify Facilities
Maintenance personnel of pump station failure.
(2) Locate the end of the spill area either in an open ditch or the point where a
storm drain empties, if required. Create a dam or insert a plug to prevent spill
from progressing further.The dam needs to be constructed near a manhole or at
a location which is accessible to the jet/vacuum truck. Build the dam using dirt or
sand-bags that are capable of containing water flow from fire hydrant(s)without
dam failure. Have backhoe at site with extra material at all times so dam can be

(3) Put up warning tape, black on red or black on yellow "Danger Do Not Enter",
from the SSO site to the pump site on both sides of the open ditch in populated
areas. In unpopulated areas restrict access where there is any chance of humans
coming in contact with the SSO.
(4) Station two (2) pumps at the dam that are capable of maintaining a pumping
rate equal to or greater than fire hydrant flow rate.Use one pump for backup.
There will be two (2)workers at the pump site at all times during pumping
operations. Pump effluent from dam into the downstream manhole and/or
vacuum truck.
5 Repair pumpstation or maintain wet well level with vacuum truck until pump

station is returned to service.
(6) Flush ditch or storm drain with water from a sufficient water source (i.e., creek
upstream of SSO or a fire hydrant)to remove wastewater from ditch or storm
draining system. Notify water treatment plant personnel prior to initiation of and
upon completion of all flushing operations.

(7) If rip rap is in ditch,wash down with high water pressure until there is no
wastewater residue remaining on, or within the rip rap structure.
(8) Continue to pump effluent from dam into the manhole or wet well until all water
has been reclaimed.
(9) Proceed to clean-up procedure (Section C).
C. CLEAN-UP PROCEDURE.
1. ASPHALT CONCRETE:
a. Set up containment at or near the storm drainage system; do not let the spill enter
the storm drainage system.
b. Put up tape, black on red or black on yellow "Danger Do Not Enter", around the SSO
area.
c. Spread lime (hydrated or pelletized) and sand on spillage (or equivalent material).
Let this stand for one (1) hour and sweep up. If moisture or grease is still present,
repeat procedure.
d. After removing the contaminated material,wash down the area with high water
pressure and dilute sodium h ochlorite solution (HTHorlorox . Pumpthis back

into the sewer system.Transport wet sand to the drying facilities at the wastewater
treatment plant.
2. PRIVATE PROPERTY:
a. Till the area whenever possible. Apply lime (hydrated or pelletized), seed and straw.
b. If not able to till, apply lime (pelletized or hydrated), and topsoil and mix thoroughly.
Lime again lightly, then seed (See Seeding Schedule) and cover with straw.
3. DITCH OR STORM DRAIN:
a. Flush ditch or storm drain with water from a sufficient source (i.e., creek upstream
from SSO or a fire hydrant)to remove wastewater from ditch or storm drain. Notify
water treatment plant personnel prior to initiation of and upon completion of all
flushing operations.
b. If rip rap is in ditch,wash down with high water pressure until there is no
wastewater residue remaining on, or within the rip rap structure.
c. Continue to pump effluent from dam into the manhole until all water has been
reclaimed.

d. Apply pelletized or hydrated lime and sand as may be required to neutralize spill
site.
