NOTIFICATION OF INTENT (NOI) TO CONSTRUCT OR OPERATE INJECTION WELLS
The following are `permitted by rule" and do not require an individualpermit when constructed in accordance
with the rules of 15A NCAC 02C.0200 (NOTE: This form must be received at least 14 DAYS prior to injection)
AQUIFER TEST WELLS (15A NCAC 02C .0220)
These wells are used to inject uncontaminated fluid into an aquifer to determine aquifer hydraulic characteristics.
IN SITU REMEDIATION (15A NCAC 02C .02251 or TRACER WELLS (15A NCAC 02C .0229):
1) Passive Injection Systems - In -well delivery systems to diffuse injectants into the subsurface. Examples include
ORC socks, iSOC systems, and other gas infusion methods (Note: Injection Event Records (IER) do not need to be
submitted for replacement of each sock used in ORC systems).
2) Small -Scale Injection Operations — Injection wells located within a land surface area not to exceed 10,000
square feet for the purpose of soil or groundwater remediation or tracer tests. An individual permit shall be required
for test or treatment areas exceeding 10,000 square feet.
3) Pilot Tests - Preliminary studies conducted for the purpose of evaluating the technical feasibility of a
remediation strategy in order to develop a full scale remediation plan for future implementation, and where the
surface area of the injection zone wells is located within an area that does not exceed five percent of the land surface
above the known extent of groundwater contamination. An individual permit shall be required to conduct more
than one pilot test on any separate groundwater contaminant plume.
4) Air Injection Wells - Used to inject ambient air to enhance in -situ treatment of soil or groundwater.
5) In -Situ Thermal Wells (IST) — Used to `heat' contaminated groundwater to enhance remediation.

monitoring wells, and existing and proposed injection wells. (See attached Figures)

and proposed wells. (See attached Figures)
I. DESCRIPTION OF PROPOSED INJECTION ACTIVITIES AT THE SITE — Provide a brief narrative
regarding the cause of the contamination, and purpose, scope, goals of the proposed injection activity:
Historically, ENCEE Chemical has reportedly stored bulk quantities of various chemicals, compounds and
solvents in drums, bulk, and pre -packaged quantities which may have contributed to shallow ,groundwater
volatile organic compounds (VOCs) and metals contamination. A pilot study is being conducted to evaluate the
effectiveness of hybrid treatment of In -situ Chemical Reduction (ISCR) and anaerobic biodegradation (ERD)
remediation in the source areas surrounding monitoring wells MW-4 and SW-2 for treating VOCs and metals in
shallow groundwater. The pilot injections will be conducted through DPT temporarinjection points (borings)
which will be abandoned after the completion of injections. The pilot study will evaluate the suitability of ISCR
and ERD for full scale remedial application. DPT injection point schematic is included in the Attachment.
J. WELL CONSTRUCTION DATA

(a) Well type as permanent, Geoprobe/DPT, or subsurface distribution infiltration gallery
(b) Depth below land surface of casing, each grout type and depth, screen, and sand pack
(c) Well contractor name and certification number

K. INJECTION SUMMARY
NOTE. Only iniectants approved by the epidemiology section of the NC Division of Public Health. Department
of Health and Human Services can be injected. Approved iniectants can be found online at
hiip: Hdeq.nc. gov/about/divisions/water-resources/water-resources-permits/wastewater-branch/ground-water-
protection/ground-water-approved-injectants. All other substances must be reviewed by the DHHS prior to use.
Contact the UIC Pro ream for more info if you wish to eft approval for a different additive. However, please
note it may take 3 months or longer. If no iniectants are to be used use N/A.
SDS sheets for the below listed Injectants are included in the Attachments. A list of Amendments
proposed for Pilot Study injection, along with their composition and comparison to NCDEQ
approved Amendments is included in the Attachment.

L. MONITORING PLAN — Describe below or in separate attachment a monitoring plan to be used to determine
if violations of groundwater quality standards specified in Subchapter 02L result from the injection activity.
Three post -injection groundwater monitoring events will be performed at a 30-day, 90-day, and 180-day
schedule after the completion of pilot injections. Groundwater samples will be analyzed for VOCs by EPA
Method 8260D, SVOCs by EPA Method 8270E, RCRA 8 Metals by EPA Method 602013, Hexavalent Chromium
by EPA Method 7196A, Nitrate/Nitrite and Sulfate by EPA Method 300.0, and MADEP EPH/VPH.

Based on Microbial Insights QuantArray° analysis, the DHC microbial consortium (SDC-9) is
comprised of microorganisms of the genera Dehalococcoides, Desulfovibrio, Desulfitobacterium,
Dehalobium, and Dehalobacter as well as sulfate reducing bacteria and methanogenic
archaebacteria.
SECTION 3 — HAZARDS IDENTIFICATION
The available data indicates no known hazards associated with exposure to this product.
Nevertheless, individuals who are allergic to enzymes or other related proteins should avoid exposure
and handling. Health effects associated with exposure to similar organisms are listed below.
Ingestion: Ingestion of large quantities may result in abdominal discomfort including nausea,
vomiting, cramps, diarrhea, and fever.
Inhalation: Hypersensitive individuals may experience breathing difficulties after inhalation of
aerosols.
Skin Absorption: May cause irritation upon prolonged contact. Hypersensitive
individuals may experience allergic reactions.
Eye contact: May cause irritation unless immediately rinsed.

SECTION 4 — FIRST -AID MEASURES
Ingestion: Thoroughly rinse mouth with water. Do not induce vomiting unless
directed to do so by medical personnel. Get immediate medical attention. Never give
anything by mouth to an unconscious or convulsing person.
Inhalation: Get medical attention if allergic symptoms develop.
Skin Absorption: N/A
Skin Contact: Wash affected area with soap and water. Get medical attention if allergic symptoms
develop.
Eye Contact: Flush eyes with plenty of water for at least 15 minutes using an eyewash fountain, if

No emergency results from spillage. However, spills should be cleaned up promptly. Absorb with an
inert material and put the spilled material in an appropriate waste disposal container. All personnel
involved in the cleanup must wear protective clothing and avoid skin contact. After clean-up, disinfect
all cleaning materials and storage containers that come in contact with the spilled liquid.

Avoid breathing breathe aerosol. Avoid contact with skin. Use personal protective equipment
recommended in Section 8.
Keep containers tightly closed in a cool, well -ventilated area. The DHC microbial consortium (SDC-9)
is typically supplied in stainless steel kegs equipped with pressure relief valves. The kegs are
pressurized with Nitrogen gas (N2) up to the pressure of 15 psi. Do not exceed pressure of 15 psi
during transfer of DHC microbial consortium (SDC-9) from kegs. Don't open keg if content of the
keg is under pressure.
DHC microbial consortium (SDC-9) may be stored for up to 4 weeks at temperature
2-40C without aeration. Avoid freezing.

Hand Protection: Rubber, nitrile, or vinyl gloves.
Eye Protection: Safety goggles or glasses with side splash shields.

SDC-9 consortium has tested negative for pathogenic microorganisms such as Bacillus cereus,
Listeria monocytogens, Salmonella sp., Fecal Coliforms, Total Coliforms, Yeast and Mold and
Pseudomonas sp.

Waste Disposal Method: No special disposal methods are required. The material is compatible with
all known biological treatment methods. To reduce odors and permanently inactivate
microorganisms, mix 100 parts (by volume) of SDC-9 consortium with 1 part (by volume) of bleach.
Dispose of in accordance with local, state and federal regulations.
