However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on February 27, 2023. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through O, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2). Please reference the subject application number when providing the requested information. Application: Item V.13 — The referenced plan sheet for the audible/visual alarms is incorrect. The alarms appear to be depicted on Sheet D-102. Please revise and add information for it to the Engineering Plan section. 2. Item V.14 — The referenced Specification page and plan sheet are incorrect for the rain sensor. Please revise the Specifications page number to "7" and clearly depict the rain sensor within the plans. Update the application accordingly. 3. Item VII.3 — The minimum observed depth to seasonal high water table (SHWT) was reported as 1 ft. Pursuant to 15A NCAC 02T .0605(p), a minimum of 18 inches of vertical separation between the apparent SHWT and the ground surface must be provided. Pursuant to 15A NCAC 02T .0605(q), a minimum of 12 inches of vertical separation between any perched SHWT and the ground surface must be provided. Please provide a document signed, sealed and dated by the application's Soil Scientist that specifies whether the reported depth is associated with the apparent or perched SHWT. The document should clarify how Items V.15 and V.16 on Form: SFRWWIS 06-16 are met according to the Soil Evaluation results. 4. Item VIIA — Please address the following: a. The area of the irrigation field does not match the provided calculations. Please revise. b. Please clarify how the Designed Rates (in/hr and in/yr) have been obtained by referring to the Engineering Calculations section or providing more calculations. c. Waterbody Stream Index No. and Classification were not provided. Please revise. d. The Drip Irrigation Design Elements table was incomplete. Please provide information regarding the designed emitter wetted area and the elevation of the highest emitter. Mr. Mackie W. Lambert January 27, 2023 Page 3 of 5 5. The application was certified by Anita Lambert; however, Ms. Lambert is not the deeded property owner, and a delegation letter was not provided. The Applicant's Certification is required to be signed in accordance with 15A NCAC 02T .0106(b). Pursuant to 15A NCAC 02T .0106(c), an alternate person may be designated as the signing official if a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T .0106(b). Please provide an Applicant's Certification signed by the deeded property owner (Mr. Mackie W. Lambert) or submit a delegation letter (Form NDSDAL 01-20) for Ms. Lambert that is signed by the deeded property owner. D. Property Ownership Documentation: 1. No comment. E. Soil Evaluation: 1. No comment. F. Engineering Plans: 1. No comment. G. Specifications: 1. No comment. H. Engineering Calculations: 1. Pursuant to 15A NCAC 02T. 0604 (c)(3), engineering calculations are required to include total dynamic head calculations and system curve analysis for each pump used. The pump curve for the field dosing pump was not included in the submittal. Please provide the necessary analysis and update Item VI.2.e on Form: SFRWWIS 06-16 to match the results of the Engineering Calculation section. I. Site Map: 1. The provided site maps (C-103 and C-104) do not include the following requirements, pursuant to 15A NCAC 02T .0604(d): a) The fences around the irrigation area. b) Soil mapping units on the irrigation site. c) Delineation of the compliance and review boundaries, pursuant to 15A NCAC 02L .0107(i) and .0108. Lambert January 27, 2023 Page 4 of 5 d) Site property boundaries within 500 feet of all wastewater treatment, storage and irrigation sites. e) All habitable residences or places of public assembly within 500 feet of all treatment, storage an irrigation sites. Please update the site maps and ensure the inclusion of all requirements in accordance with 15A NCAC 02T .0604(d). 2. The site maps do not include associated setbacks for the proposed dwelling, the surface water (existing stream), and the water line. Please update the site maps to include all required setbacks, pursuant to 15A NCAC 02T .0606. 3. On the site maps, the setbacks associated with the property line and the proposed wells do not match the required setbacks, pursuant to 15A NCAC 02T .0606. Please submit the site maps with corrected setbacks and associated notes. J. Operation and Maintenance Plan: 1. No comment. K. Operation and Maintenance Agreement: A review of the Chatham County Register of Deeds shows that Parcel No. 79888 is solely owned by Mackie W. Lambert. The Permittee names on the Operation and Maintenance Agreement (Anita & Matthew Lambert) do not match the name of the deeded property owner. Additionally, the signature on the provided copy was illegible due to poor resolution of the scan. Please provide a revised Operation and Maintenance Agreement that has been completed and signed by Mr. Mackie W. Lambert. L. Existing Permit: 1. Not applicable. M. Threatened or Endangered Aquatic Species Documentation: The Threatened and Endangered Aquatic Species Documentation in this application provided by the North Carolina Natural Heritage Program shows that there are records of rare species within the proposed project boundary. Pursuant to 15A NCAC 02T .0105(i), Non -Discharge permits shall provide specific conditions to address the protection of threatened or endangered aquatic species, as provided in plans developed pursuant to 15A NCAC 02B .0110. Please determine whether the construction and operation of the facility directly impacts the identified aquatic species and identify site -specific strategies to maintain or recover the water quality conditions to meet the requirements of 15A NCAC 02B .0110, if necessary.
