Dear Geoff,
Groundwater Management Associates, Inc. (GMA) is working as a subconsultant to The Wooten
Company to support the construction phase of ONWASA's Southeast Area Infiltration Basin
wastewater treatment facility near Holly Ridge, Onslow County, NC. This letter includes an
Application for a Permit to Construct a Water Supply Well (Form GW-22W) that will be used in
the filling and testing of infiltration basins for the facility. The completed application form is
attached along with figures and other supporting documentation. Below, we provide details on
the proposed well that will withdraw from the Castle Hayne Aquifer.
Background
The new wastewater treatment facility will include a series of 26 High Rate Infiltration (HRI)
basins to manage the infiltration of high -quality effluent from a new wastewater treatment plant
that will be built at a site on Dolph Everett Road (see Figure 1 and Attachment 1). The HRI
basins will operate under an amended Non -Discharge permit for wastewater effluent disposal
(Permit #WQ0019907).
Prior to use of the HRI basins, each basin will be tested by filling it with non -potable water
produced from a new water -supply well that is planned to be installed at the facility. The
proposed well will be screened within the lower Castle Hayne Aquifer unit, and the design
capacity for the well will be 400 gallons per minute (GPM). The new well will be used to fill
each HRI basin individually.

The Wooten Company anticipates that construction of the well will occur in 2024. Withdrawals
from the well will be covered by a Central Coastal Plain Capacity Use Area permit, and ONWASA
is applying for the CCPCUA permit. This well will be used for non -potable purposes, and this
will not be a Public Water Supply Well.
Well Construction Permit Application Details
The completed GW-22W form (attached) provides details of the well owner, contact
information, intended use, and design capacity for the well. The Wooten Company is designing
the pipeline connections to direct water from the well to fill each new infiltration basin and to
conduct individual controlled infiltration tests. This testing is required before the new infiltration
basins can accept treated wastewater.
The Attached Figure 1 presents the location for the proposed well in addition to property lines.
The proposed well location will be at N 34.5065940, W 77.5015810. The proposed well location
meets the setback requirements outlined in 15A NCAC 02C .0107.
Figure 1 also illustrates the locations of four residential properties within 1500 feet of the
proposed well. Those residential properties are not served by public water supply from
ONWASA, so all of those properties are believed to be supplied by individual private wells. The
closest of these properties is approximately 500 feet from the proposed well. No other supply
well sites occur within 1500 feet of the proposed well.
Figure 2 presents a diagram showing the proposed well construction details, including: borehole
and well diameters, planned well depth, screen intervals, gravel -pack interval, casing materials
and thicknesses, grout intervals, and wellhead completion details. Note that the well casing will
extend a minimum of 12 inches above land surface and will be equipped with a temporary well
seal until the final pump design and wellhead piping can be determined by The Wooten
Company. Following well construction and development, a 24-hour constant rate pumping test
will be performed at the 400 gpm design capacity. Based upon the hydraulic performance of
the well as determined by the pumping test, a final pump will be designed, and the final
wellhead will be constructed.
At this time, The Wooten Company has not bid the drilling work. All well contractor work will
be done by a NC Licensed Well Contractor, and all well construction will be completed in
accordance with the 15A NCAC 02C Well Construction Standards. The Wooten Company and
ONWASA anticipate that construction of the well will not begin for approximately 15 months
(estimated April of 2024). The well construction work will be performed as a part of a larger
construction project that will include the pump installation and piping system to convey water
from the proposed well to the infiltration basins for infiltration testing. Obtaining a well
construction permit is needed now as a part of obtaining an updated Non -Discharge Permit

from NCDEQ. So, ONWASA is applying for the well construction permit in advance of
contracting for the work.
The design flow rate of the proposed well is 400 GPM, and the daily capacity of the well will be
0.576 MGD. A Central Coastal Plain Capacity Use Area permit will be required for the proposed
use of the well. A CCPCUA permit application is being submitted to the NC Division of Water
Resources, and well construction will not occur until the CCPCUA permit has been received.
The proposed well location is within the footprint of an existing spray irrigation field that will be
decommissioned prior to construction of the new well. As shown on Attachment 1, the
proposed non -potable water -supply well will meet the setback of > 100 feet from the proposed
infiltration basins that will be constructed. GMA has also reviewed NCDEQ incident maps of the
area, and we did not identify any known or suspected contamination sources within 500 feet of
the proposed well.
