Box 2736 Winston-Salem, NC 27102-2736 SUBJECT: Compliance Evaluation Inspection Former Coliseum Shell NPDES #: NCO088919 Forsyth County Dear Mr, Rhymer: Ron Boone, of the Winston-Salem Regional Office (WSRO) of the NC Division of Water Resources (DWR or Division), conducted a compliance evaluation inspection (CEI) of the ground water remediation plant at the Former Coliseum Shell on November 17, 2015. The assistance and cooperation of Ryan Kerins, Operator in Responsible Charge (ORC) of the treatment plant, was greatly appreciated. The inspection is summarized below and an inspection report is attached for your records. General Information The Former Coliseum Shell is located at 3009 University Parkway, Winston Salem, Forsyth County, NC. The approximate coordinates of the treatment building are 36.1291280N,-80.261420°W. The permit authorizes Quality Oil Company, LLC to operate this 0.036 MGD groundwater remediation system and discharge the treated effluent into an unnamed tributary to Silas Creek via outfall 001. Silas Creek is currently classified as Class C waters in the Yadkin Pee Dee River basin. Review Mr. Kerins informed Mr. Boone by phone that the treatment plant was shut down August 31, 2015, and that no restart of the plant is scheduled at this time. Therefore, for this inspection, Mr. Boone simply performed a file review and a review of the Discharge Monitoring Reports we have on file for the facility. No on site inspection of the facility was performed. Mr. Boone noted no concerns, discrepancies, or permit violations during this file review. Please inform Mr. Boone before the treatment plant is brought back on line so an onsite visit can be made.
