Alphin, Jr. Facility: Scarlett Acres Mobile Home Park Wastewater Treatment Plant NPDES Permit#: NCO061204 Forsyth County Dear Mr. Alphin: Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Resources (DWR or the Division) conducted a compliance evaluation inspection (CEI) of the Scarlett Acres Mobile Home Park Wastewater Treatment Plant on November 18, 2015. Your assistance and cooperation during the inspection, as well as that of Randy Bell, Operator in Responsible Charge, was greatly appreciated. An inspection checklist is attached for your records and inspection findings are summarized below. General Information 2. The wastewater treatment plant (WWTP) is located at 5528 Sherene Lane, in Walkertown, Forsyth County, NC, at approximate coordinates 36.168276°,-80.181438°. The permit authorizes Scarlett Acres to operate this 0.02 MGD WWTP, which consists of an equalization basin with influent pump station, a bar screen, flow splitter box, two package plants with aeration basins and fixed media in the northern most basin; two clarifiers, a disinfection unit, post aeration, a dechlorination unit, and an aerated digester. Scarlett Acres is further authorized to discharge the treated effluent from this WWTP via outfall 001, which is located at approximate coordinates 36.1675000,-80.1850000, into an unnamed tributary to Mill Creek, which is currently classified as Class C waters in the Yadkin Pee Dee River Basin. Site Review 3. The entire plant was inspected. The flow meter has been down since approximately October 15th'and they haven't been able to have it repaired yet. They hope to have it back up and running within the next month or so. The flow meter was last calibrated on May 8, 2015. The flow meter must be repaired as soon as possible. Instantaneous flow readings may be taken in the interim in order to have some sort of flow measurement to put on the discharge monitoring reports. Please note that something must be done to minimize the amount of time that the flow meter is down when it malfunctions. If the delay, this time, was due to a lack of parts, then spare parts should be kept on hand to prevent long downtimes in the future. If the problem was unavailability of a vender capable of repairing the meter, then a vender who can repair it expediently must be found and placed on call. The flow meter should never be down for an extended period of time. This constitutes a violation of the permit condition to provide proper operation and maintenance of the plant at all times. A new one is being built. Grinder pumps have been installed in the equalization basin in the interim. The Division was never notified of the failure, removal, and replacement of the bar screen. The permit requires that the permittee notifies the Division when a plant component fails, as well as when a plant component is replaced with something different; the Division must approve the replacement component. This is normally done with an Authorization to Construct Permit. Mr. Boone noted no further discrepancies or violations. Documentation Review 4. All documentation was reviewed, including Discharge Monitoring Reports (DMR), laboratory and field laboratory records, calibration data, chains of custody, operation and maintenance logs, operator visitation logs, etc. All documentation was complete and current, 5. Please reply to this Notice of Violation in writing within 10 business days of receiving it. Your written reply should provide all details of the actions taken and that you plan to take to fully address the issues noted above in paragraph 3. Under no circumstances should the flow meter go inoperable beyond the end of November, 2015. The new bar screen should also be installed no later than November 30, 2015. If you foresee these deadlines to be a problem, then you should call our office and schedule a meeting to fully discuss this issues and develop workable solutions. 6. You are reminded that violations of the NC0061204 permit and the NC rules and/or regulations that implement it, are subject to civil penalty assessments of up to $25,000 per day, per violation.
