ADDRESS: 5915 Star Lane Houston, TX 77057 CERTIFICATE NO: #617 DATE OF INSPECTION: 5/19/2006 TYPE OF INSPECTION: Maintenance EVALUATOR: Mr. David G. Livingston LOCAL PERSON(S) CONTACTED: Mr. Daniel Zabihi and Mr. Robert McDonald I. INTRODUCTION: This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for the analysis of environmental samples. II. GENERAL COMMENTS: The laboratory analyzes Oil samples. The laboratory is spacious and well equipped. All facilities and equipment are well maintained. Records are well kept and most data looked good. Some further quality control procedures need to be implemented. III. DEFICIENCIES, REQUIREMENTS, COMMENTS AND RECOMMENDATIONS: NORTH CAROLINA LABORATORY CERTIFICATION (NCLC) PROGRAM POLICIES NCLC Policy: At any time a laboratory receives samples which do not meet sample collection, holding time, or preservation requirements, the laboratory must notify the sample collector or client and secure another sample if possible. If another sample cannot be secured the original sample may be analyzed, but the results reported must be qualified with the nature of the infraction(s) and the laboratory must notify the State Laboratory about the infraction(s) . The notification must include a statement indicating corrective action taken to prevent the problem for future samples. Ref. 15A NCAC 2H .0805 (a) (7) (n) NCLC Policy: Whenever a certified commercial laboratory refers or subcontracts samples to another certified laboratory for analyses, the referring laboratory must supply the date and time samples were collected to insure holding times are met. Subcontracted samples must clearly indicate the State of North Carolina as the collection site on all record transcriptions. Laboratories may subcontract sample fractions, extracts, leachates and other sample preparation products provided that all Rules and requirements of 15A NCAC 02H .0800 are documented. The initial client requesting the analyses must receive the original or a copy of the report made by the laboratory that performs the analyses. Ref: 15A NCAC 2H .0805 (e) (2) METALS (1) DEFICIENCY: The laboratory does not have a second source QC standard for this parameter. Page 2 Precision Petroleum Labs, Inc. REQUIREMENT: Excluding Oil and Grease, all residue parameters, TCLP extractions, residual chlorine, and coliform, analyze one known standard in addition to calibration standards each day samples are analyzed to document accuracy. Analyze one suspended residue, one dissolved residue, one residual chlorine and one oil and grease standard quarterly. For residual chlorine, all calibration standards required by the approved procedure in use and by the EPA must be analyzed. Ref: 15A NCAC 2H .0800(a) (7) (b). Note: The laboratory has only been able to find one source for standards, Constan. The lab needs to order a different lot # to meet the second source standard requirement. (A) COMMENT: The laboratory has a digestion log book but does not currently document the Lot # of the acid used in the digestion. RECOMMENDATION: Document the acid Lot # used to digest samples in the digestion log. ORGANICS (B) COMMENT: The laboratory’s stated calibration limits, in the SOP and OA manual, were reversed for GC and GC/MS methods. The laboratory had an acceptable %RSD set at 15% for GC methods and acceptable %RSD set at 20% for GC/MS methods. REQUIREMENT: The 20% RSD criteria in Method 8000 pertains to GC and HPLC methods other than GC/MS. Method 8260 requires 15% RSD. Ref: SW-846 Method 8260B 7.3.8.3 Note. REQUIREMENT: Update calibration limits in your SOP and QA manual. IV. PAPER TRAIL INVESTIGATION: This consisted of comparing data with the values obtained on laboratory bench worksheets. V. CONCLUSIONS: Correcting the above cited deficiency and comments and implementing the requirements and recommendations will help this lab to produce quality data and meet certification requirements.
