This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for
the analysis of environmental samples.
The laboratory was clean and well organized. The facility has all the equipment necessary to perform the
analyses. Technical Assistance and Quality Assurance Policies were provided at the time of the
inspection. This laboratory only analyzes temperature. A contract laboratory analyzes pH, Temperature,
Dissolved Oxygen, and Total Residual Chlorine.
Documentation
Comment: The benchsheet combines both the laboratory and contract laboratory data. For each
parameter, a time is documented with no indication of it being collection or analysis time. The North
Carolina Administrative Code, 15A NCAC 2H .0805 (g) (1) requires, Data pertinent to each analysis must
be maintained for five years. Certified Data must consist of date collected, time collected, samples site,
sample collector, and sample analysis time. The field bench sheets must provide a space for the
signature of the analyst, and proper units of measure for all analyses. On March 4, 2010, a new
benchsheet was provided to record all required information. The analyst stated that the revised
benchsheet will be put into use by March 12, 2010. No further response is necessary for this
finding.
Comment: When the contract laboratory performs its analyses, two temperature results are recorded;
one by the contract laboratory and one by laboratory staff. The temperature recorded by the contract
laboratory is not reported on the Discharge Monitoring Report (DMR). The North Carolina Administrative
Code, 15A NCAC 2B .0506 (b) (3) (J) requires, The results of all tests on the characteristics of the
effluent, including but not limited to NPDES permit monitoring requirements, shall be reported on the
monthly report forms. The analyst stated that, in the future, temperature would not be analyzed by him
on days that the contract laboratory is on site sampling. No further response is necessary for this
finding.

Comment: Error corrections were not properly made. Some instances of scribbling out errors and
correction fluid use were noted. The North Carolina Wastewater/Groundwater Laboratory Certification
document, Quality Assurance Policies for Field Laboratories, states all documentation errors must be
corrected by drawing a single line through the error so that the original entry remains legible. Entries shall
not be obliterated by erasures or markings. Wite-Out®, correction tape or similar products designed to
obliterate documentation are not to be used. Write the correction adjacent to the error. The correction
must be initialed by the responsible individual and the date of change documented. All data and log
entries must be written in indelible ink. Pencil entries are not acceptable. The analyst stated that he
understood the policy and would implement it immediately. No further response is necessary for this
finding.
Comment: Quality Control failures, reported to the laboratory from the contract laboratory, were not
reported on the DMR. The North Carolina Wastewater/Groundwater Laboratory Certification document,
Quality Assurance Policies for Field Laboratories, states in part, where applicable, a notation must be
made on the Discharge Monitoring Report (DMR) form, in the comment section, when any required
sample quality control does not meet specified criteria and another sample cannot be obtained. The
analyst was unaware of this requirement and stated that this information would be included on DMRs in
the future. No further response is necessary for this finding.
Temperature — Standard Methods, 18" Edition, 2550 B
Comment: The temperature correction was not posted on the thermometer. The Technical Assistance
for Field Analysis of Temperature states that the temperature correction (even if it is zero) must be posted
on the meter as well as in hard copy format (to be retained for 5 years). The temperature correction was
posted on the thermometer at the time of the inspection. No further response is necessary for this
finding.
IV. PAPER TRAIL INVESTIGATION:
The paper trail consisted of comparing field testing records and contract lab reports to Discharge
Monitoring Reports (DMRs) submitted to the North Carolina Division of Water Quality. Data were
reviewed for Clay County Water & Sewer District WWTP (NPDES permit #NC0026697) for May and
June, 2009. No transcription errors were detected. The facility appears to be doing a good job of
accurately transcribing data.
V. CONCLUSIONS:
All of the findings noted during the inspection were adequately addressed prior to the completion of this
report. The inspector would like to thank the staff for its assistance during the inspection and data review
process. No response is required.
