Morgan St., Room #25 Roxboro, NC 27573 CERTIFICATE #: 5397 DATE OF INSPECTION: August 24, 2010 TYPE OF INSPECTION: Field Maintenance AUDITOR(S): Jeffrey R. Adams LOCAL PERSON(S) CONTACTED: Paul Phillips and Chris Clayton I. INTRODUCTION: This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for the analysis of environmental samples. II. GENERAL COMMENTS: The laboratory was neat and equipment was well maintained. Records were maintained and well organized. The only finding cited was corrected during the inspection. The laboratory was given a packet containing the Technical Assistance Documents and quality control requirements for all field parameters during the inspection. III. FINDINGS, REQUIREMENTS, COMMENTS AND RECOMMENDATIONS: Dissolved Oxygen – Standard Methods, 18th Edition, 4500 O G Comment: Proper units of measure (i.e., mg/L for Dissolved Oxygen) were not documented on the benchsheet. The North Carolina Administrative Code, 15A NCAC 2H .0805 (g) (1) states: Data pertinent to each analysis must be maintained for five years. Certified Data must consist of date collected, time collected, sample site, sample collector, and sample analysis time. The field bench sheets must provide a space for the signature or initials of the analyst, and proper units of measure for all analyses. This benchsheet correction was made during the inspection. The laboratory is now recording the units of measure on the benchsheet. No further response is necessary for this finding. IV. PAPER TRAIL INVESTIGATION: The paper trail consisted of comparing field testing records and contract lab reports to Discharge Monitoring Reports (DMRs) submitted to the North Carolina Division of Water Quality. Data were reviewed for the Person County Board of Education (NPDES permit #NC0036536) for March, May, June and July, 2010. No transcription errors were detected. The facility appears to be doing a good job of accurately transcribing data. V. CONCLUSIONS: All findings noted during the inspection were adequately addressed prior to the completion of this report.
