I am following the six step process and will be mailing a hardcopy of the Stormwater eReporting Registration Form to the
address provided on the website. While following Step 1 (check information in the Owner Verification Report and SW
Permit Summary Report), I wanted to ask you about 2 updates needed on the SW Permit Summary Report to see if you
can help with this.

Updates;

1- Address under Facility Contact Persons (Page 1)-
This address is listed as 401 E. Church St., Edenton, NC 27932. That was an old address and should be updated
to 125 Dye Plant R., Edenton, NC 27932. That is the facility address and mailing address for the contacts and
owners.

2- Outfall (Page 2)-
There is only one outfall associated with the permitted activity which is the boat ramp and is the only outfall
that has been sampled since issuance of the permit. We had discussed this with the Washington Regional Office
rep several years ago and it was understood that 2 other former outfalls are not associated with this barge
maintenance facility

Can you please assist with updating this form?
