However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on November 2, 2022. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through D, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2). Please reference the subject application number when providing the requested information. High -Rate Infiltration System — Renewal (FORM: HRIS-R 02-21): 1. The open -atmosphere structure in Section VI was not completed for the five open -atmosphere high -rate infiltration basins. This information shall be provided in an updated application to verify their locations. 2. Section VIII — Monitoring Wells lists MW-4, MW-5 and MW-6. The Attachment C of the previous permit includes MW-1, MW-2 and MW-3 as well. This section shall be updated in a revised application form. 3. The sites in Section IX — Infiltration Sites only mention basins 4 and 5. The Attachment B of the previous permit includes sites 1, 2 and 3. This section shall be updated in a revised application form. 4. The deeded owners to the sites in Section IX are listed as Old North State Water Company, LLC, but the deeded owner according to the Onslow County GIS website is A Sydes Construction, Inc. This section shall be updated with this revised information. B. Attachment G — Easement, Encroachment, and Lease Agreements: 1. See Item A.5. It is indicated in this section that the Permittee owns all land associated with the system, but the Deeded Owners listed in Section IX is not the Permittee. A copy of all easements, lease agreements, and encroachment agreements allowing the Permittee to operate and maintain the system on property not owned by the Permittee shall be provided in accordance with 15A NCAC 02T .0116(c). C. Attachment I — Compliance Schedules: 1. Attachment I requests documentation that compliance schedules in Section I of the most recently issues permit have been met. It was indicated that there were no compliance schedules, but the following compliance schedules were included in the previous permit and documentation was not found verifying they were completed: a. Condition I.3. -A Residuals Management Plan that meets the requirements of I5A NCAC 02T .07046) and .0708 shall be prepared and available for review by the Division prior to facility startup. The Operation and Maintenance Plan was furnished as required by Condition I.2., but this plan didn't appear to include a section for residuals management. b. Condition I.5. — Upon completion of construction and prior to operation of the high -rate infiltration basins, each basin shall be inspected by a licensed Professional Geologist (PG) to veri that the constructed basins will be capable of receiving the wastewater at the permitted loading rates. There was one evaluation by a licensed PG found for Basin 4 which certifies that the basin will accommodate 75,000 gallons per day. However, these evaluations are not included for basins 3 and 5. Documentation verifying that these compliance schedules have been met shall be provided. D. Existing Permit: 1. Condition IV.11. of the previous permit required the Permittee to submit a Well Construction Record Form (GW-1) for initial sampling of monitoring wells. Please provide the GW-1 for any monitoring well that has been constructed and is actively sampling.
