Dear Mr. Farrell:
On March 7, 2007, personnel with the Division of Water Quality (DWQ) reviewed the completed above-
referenced project for as-built compliance. The project was not installed per the specifications of the plan that
was approved by the DWQ on June 6, 2006. A corrective action plan was discussed with you during the
March 7, 2007, field visit. This corrective action plan requires you to reconfigure the stream segment and bank
stabilization immediately downstream of the culvert per the originally permitted plan as shown on Sheet No. 2,
titled "Grading Plan", dated March 1, 2006, and received by the DWQ on Apri121, 2006.
Please contact the DWQ within three, weeks of the date of this letter to verify that you have received this letter,
and please provide us with a timetable of when the requested corrective action plan will be completed. If we
do not hear from you within three weeks, we will move toward an enforcement action for the project.
This letter only addresses the as-built review and does not authorize any impacts to wetlands, waters or
