A Compliance Evaluation Inspection was performed on February 25, 2015 by George Smith. The inspection is
comprised of an overall facilities site evaluation and to determine if the wastewater treatment plant was constructed.
Permit
Your NPDES permit became effective on July 1,2011 and expires on March 31, 2016.
Facility Site Review
The location of the site: off Hwy 421, go 5.0 miles on Hwy 194 N, right on Big Hill Rd, 1st right on Courtney Taylor
Lane.
On April 8, 2014 I spoke with Kim, and she is the main contact person for this permit. She can be contacted at 828-
963-3741.
There are two buildings on the site, each with 4 units. These units are permitted through the Appalachian Health
Department. It is a pre-treat using peat moss pods to 25%reduction system.
The compliance evaluation inspection is satisfactory. Please remember that you must apply for and receive an
Authorization to Construct before you proceed with a wastewater treatment design. If you have any questions please
