SEP 3 0 2014 Facility: Flakt Products Remediation Site Water Duality NPDES Permit#: NC0085871 Permitting Section Forsyth County Dear Ms. Christopher: Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ or the Division) conducted a compliance evaluation inspection (CEI) of the Flakt Products Remediation Site on September 23, 2014. The assistance and cooperation of Glenn Price, Operator in Responsible Charge (ORC), was greatly appreciated. An inspection checklist is attached for your records and inspection findings are summarized below. General Information The Flakt Products Remediation Site is located at the former Flakt Products manufacturing facility at 2000 Lowery Street, Winston Salem, Forsyth County, NC. The approximate coordinates of the treatment building are 36.091743°N, -80.215377°W. It is located on the southwest corner of the facility. The permit authorizes Flakt Products, Inc. to operate this 0.0864 MGD groundwater remediation system, which consists of extraction wells, an iron sequestering feed pump, an equalization tank with feed pump, an air stripper, and a flow monitoring device. The permit authorizes Flakt Products, Inc. to discharge the treated effluent into Brushy Fork via outfall 001. Brushy Fork is currently classified as Class C waters in the Yadkin Pee-Dee River basin. Site Review Mr. Price has done a good job of operating and maintaining the plant. The entire plant was clean and appeared to be well maintained. The right-of-way to the outfall was clear and ready for inspection. Mr. Boone noted no discrepancies or permit violations. Documentation Review All required documentation was reviewed. All of Mr. Price's records were available, in order, complete and current; this includes operations and visitation logs, discharge monitoring reports, laboratory records, chains of custody,etc. No discrepancies were noted.
