On December 13, 2022, the Division of Water Resources (Division) received your application requesting a 401 Water
Quality Certification from the Division for the subject project, with the application review fee received January 10, 2023.
Additional information in support of the project was received January 17, 2023. The Division has determined that your
application is incomplete and cannot be processed. The application is on -hold until all of the following information is
received:
1. The proposed plans include a temporary stream bypass to be constructed with matting to divert stream flow
during culvert installation. Typically, temporary stream diversions are lined with poly -fabric rather than matting.
Please revise the plans to specify poly -fabric for the temporary stream diversion or provide calculations that
demonstrate the peak runoff from the 10-year storm will not exceed the allowable velocity or shear stress for
the specified matting product. [15A NCAC 02H .0506(b)(2) and (3)]
Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the
proper consideration of the application. Please provide your response by February 17, 2023. If all of the requested
information is not received, the Division will be unable to approve the application and it will be denied as incomplete.
The denial of this project will necessitate reapplication to the Division for approval, including a complete application
package and appropriate fee.

Hi Andrew,
Admittedly, this is my first time submitting electronically in North Carolina so I apologize if those attachments didn't
come through. I've added the requested information here for your review:
1) I've attached an agent authorization form (#4) completed by the owner allowing entry to the site for the USACE
and NCDWR. I've also attached an agent authorization form (#5) completed by Steve Boone of Buckeye
Community Hope Foundation allowing ECS to act on their behalf.
2) I've attached the set of maps prepared during our delineation of the site (#10). I also have a set of site plans
prepared by the civil engineer for the project but the file size is around 30mb. Do you have an upload link I could
use to provide those plans? I can upload to google drive on our end but not sure if that would pose a security
issue for your agencies.
Please let me know if you require anything in addition to the items above. I'm available by phone as well if that's easier,

must be provided. Please provide an agent authorization form signed by a representative of Buckeye

2. No maps or other figures were provided with the Pre -Construction Notification. Please provide a map that
delineates the boundaries of the land proposed to be utilized by the applicant in carrying out the activity; the
location, dimensions, and type of any structures erected or to be erected on the lands for use in connection with
the activity; and the location and extent of the receiving waters, including wetlands within the boundaries of the
lands. [15A NCAC 02H .0502(a)(9)]
