5. Activities and Impacts

b. Give a brief description of purpose, use, and daily operations of the project when complete.
Project will protect lot's exposure to Albemarle Sound from ongoing erosion with a bulkhead 2-ft waterward of existing
alignment; provide a private, non-commercial, 3-slip boat docking, boatlift, boathouse and swim platform capacity in support
of traditional waterfront residential uses, along with potential for leasing 6 mooring slips; and restore/improve capacity for
boatramp utilization by supplementing armor riprap in same footprint, & including a launch access road and dock.
c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it is to be stored.
Protect shoreline landward of bulkhead with silt fence; provide clean soil/sand from upland sources for bulkhead backfill;
set bulkhead piles and sheet piles with excavator equipped vibratory hammer, and or water jet; place backfill with excavator;
set pier, boathouse, boatramp launch access dock, and swim platform piles with barge mouted vibratory hammer and/or water
jet; construct pier, platform, boathouse and walkways; add riprap to edges of boatramp by excavator and hand placement;
construct boatramp access road; finish grade and seed disturbed areas; store equipment on uplands when not in use.
d. List all development activities you propose.
Construct replacement bulkhead 2-ft waterward of existing alignment; truck -in from offsite source and grade adjoining uplands
for clean soil/sand as backfill; replace existing pier; construct swim platform at end of pier; construct 6 mooring slips at end of
pier, landward of swim platform, including 6 finger piers & 8 mooring piles; construct boathouse alongside pier with single
boatlift and finger pier on one side; construct 2 jet -ski boatlifts with finger pier extending from main pier; supplement riprap
armor at sides of boatramp & equip with launch access dock; and, construct boatramp access road.

6. Additional Information
In addition to this completed application form, (MP-1) the following items below, if applicable, must be submitted in order for the application
package to be complete. Items (a) — (f) are always applicable to any major development application. Please consult the application
instruction booklet on how to properly prepare the required items below.
a. A project narrative.
b. An accurate, dated work plat (including plan view and cross -sectional drawings) drawn to scale. Please give the present status of the
proposed project. Is any portion already complete? If previously authorized work, clearly indicate on maps, plats, drawings to distinguish
between work completed and proposed.
c. A site or location map that is sufficiently detailed to guide agency personnel unfamiliar with the area to the site.
d. A copy of the deed (with state application only) or other instrument under which the applicant claims title to the affected properties.
e. The appropriate application fee. Check or money order made payable to DENR.
f. A list of the names and complete addresses of the adjacent waterfront (riparian) landowners and signed return receipts as proof that such
owners have received a copy of the application and plats by certified mail. Such landowners must be advised that they have 30 days in
which to submit comments on the proposed project to the Division of Coastal Management.
