This letter is in response the Notice of Violation — NOV-2022-MV-0168
During the month of October we forgot to sample for our monthly Total Nitrogen and
Phosphorus sample. We were scheduled to sample for the Total Nitrogen and Phosphorus on
Wednesday October 12th.
Our Lab operator/back up ORC Chris Heedick went to the doctor on October 10th and was
diagnosed with cancer. This was a sudden diagnosis so the days after that he was on the phone
with doctors and scheduling scans and appointments so he just missed sampling for the
Nitrogen and Phosphorus. He usually double checks the following Wednesdays and at the end
of the month but since he was out so much our back up lab operator was doing the sampling.
Since he was still in training he was under the impression that he didn't need to sample for
anything else so he just sampled for our regular samples.
We have worked with him this past week and we showed him how to look at our chain of
custody papers from the week before to catch anything that one of us might have missed. We
have also printed out a monthly calendar where we write what things we are sampling for on
specific days, like Tox or Phosphorus, so we can look at that before we fill out or chain of
custody and the person collecting the samples will know if there is any additional samples that
need to be collected. We will also check off on the calendar what samples we have done so we
can have a visual record of what we have sampled for.
