The purpose of this letter is to request a modification to the United States Army Corps of Engineers
(USACE) Individual Permit and North Carolina Division of Water Resources Certification for the above
referenced project. The NCDOT is submitting for approval the installation of temporary stream crossings
for the purpose of providing access for the construction of iwo bridge structures over Buffalo Creek at
approximate Sta. 143+00 L(Permit Site 37), resulting in 200 linear feet (100 linear feet per crossing) of
temporary stream impacts. One of the temporary crossings may have to be relocated further upstream of
the proposed bridge but will still have the same linear feet of impaci. Without a stream crossing, there is
no way to access the area adjacent to the bridges between the South embankment and Buffalo Creek, a
distance of over 500 feet. (please refer to the attached Permit Drawing Sheet 38A of 93). Table 1 has
been revised to reflect the new Permit Site 37.
The proposed piped crossing has been detailed to accommodate normal low to moderate flow of Buffalo
Creek and allow overtopping with the occurrence of larger rainfall events. Upon completion of bridge
