On January 19, 2023 a site inspection was conducted for the Thomas Concrete of Carolina, Inc. facility
located at the 507 Old Montague Ln_, Wake County, North Carolina. A copy of the Compliance Inspection
Report is enclosed for your review. The site visit and file review revealed that the subject facility is
covered by NPDES Stormwater General Permit-NCG140000, Certificate of Coverage-NCG140418.
Permit coverage authorizes the discharge of stormwater from the facility to receiving waters designated
as Milburnie Creek, a Class C;NSW waterbody in the Neuse River Basin.
As a result of the inspection, the facility was found to be not in compliance with the conditions of the
NCG140000 permit. Please refer to the enclosed Compliance Inspection Report for additional comments
and observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil
penalty assessment of up to $25,000 per day for each violation. If you or your staff has any

Inspection Summary:
In order to maintain the stormwater pollution prevention plan (SP3)it is important to update the plan annually. The plan
appears to have only been updated in 2018 and again in 2022.
Be sure to update the spill log at least yearly if there are no spills by noting no spills for the year.
Train all personnel and the actions to take if there is a spill at the facility. Also be sure that personnel responsible for spill
response are aware of what quantity is reportable to DEQ.
There is a low area on the SW side of the facility that drains stormwater. The inspection report from 2015 noted the low area
and equested that a berm or other means be taken to raise that area. Raise the grade in that area or designate that as an
outfall area and request a permit update for your permit to include analytical sampling.
The material drying area along the West side of the facility has a section of the retaining wall missing on the South side that
is discharging stormwater that needs to be replaced. Replace that section of the wall or include that area as an outfall area
and begin to sample that outfall
The preventive maintenance and good housekeeping section of the SP3 does not cover all the major pollution sources on
site and should be updated. The conex trailer stores drums of chemicals or oils and was open at the time of the inspection.
That storage container should remain closed when not in use and it should be included in the semiannual site inspection.
The fuel despenser beside the diesel AST should be included in the preventive maintenance and good housekeeping portion
of the SP3 as a major pollution source. That area should also be included in the semiannual site inspection report to be
inspected at least semiannually.
