Box 368 Thomasville, NC 27360 PERMITS & ENGINURING SUBJECT: Permit No. NC0024112 Authorization to Construct City of Thomasville Wastewater Treatment Improvements EPA Project No. C370619-01 Dear Mayor Myers: The final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of the 4 MGD Hamby Creek Wastewater Treatment Plant Improvements to consist of two (2) new aeration tanks, upgrade four (4) existing aeration tanks, upgrade two (2) existing final clarifiers with skimming equipment, one (1) new final clarifier, upgrade activated sludge pump station including three (3) 835 gpm sludge pumps and wet well, chemical feed facilities and caustic tanks for alkalinity control, 1.0 meter belt press facility, Junction Box No. 4 and No. 5, 200 gpm variable speed sludge transfer pump, 2800 gpm lagoon standby pump, instrumentation and laboratory expansion, blower building additions including a 3000 cfm blower and air piping, influent and effluent sampling pumps, yard piping, grading and electrical work. Do not proceed with construction until the Authorization -to -Award package, EEO, and MBE documentation/certification has been reviewed, and you are in receipt of our approval if Federal Funding is desired for project construction. Myers August 20, 1985 Page 2 The Permittee shall employ an actively State certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. Such operator must hold a valid State certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. When a new chief operator must be obtained to be in responsible charge of the new facilities, it is required that this be done by the time the construction of the facili- ties is 50% completed. This Authorization -to -Construct is issued in accordance with Part III, Paragraph C of NPDES Permit No. NC0024112 issued July 1, 1984, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the approved plans, specifications, supporting documents, and are in compliance with the conditions and limitations specified in Permit No. NC0024112. During the construction of the proposed additions/modifications, the Permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at all times, and in such manner, as necessary to comply with the effluent limits specified in the NPDES Permit. You are reminded that it is mandatory for project to be constructed in accordance with the North Carolina Sedimentation Pollution Control Act; and when applicable, the North Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractors responsibilities shall be in complying with these acts. The selling or free distribution of wet, dried, composted sludge or sludge like materials from wastewater facilities without prior review and approval by this Division is illegal. One (1) set of final approved plans and specifications is to be forwarded to you, one (1) set to the Environmental Protection Agency (EPA), and two (2) sets to the Corps of Engineers.
