Dear Mr. McCoy:
Enclosed is a report for the inspection performed on March 5, 2014 by Nick Jones. Since the
finding(s) cited during the inspection were all corrected prior to the completion of the enclosed
report, a response is not required. The staff is commended for taking the initiative in correcting the
findings in such a timely manner. For certification maintenance, your laboratory must continue to
carry out the requirements set forth in 15A NCAC 2H .0800.

This laboratory was inspected by a representative of the North Carolina
Wastewater/Groundwater Laboratory Certification (NC WW/GW LC) program to verify its
compliance with the requirements of 15A NCAC 2H .0800 for the analysis of
environmental samples.
II. GENERAL COMMENTS:
The laboratory was clean and well organized. The facility has all the equipment necessary
to perform the analyses. The system for traceability of standard and reagent preparation is
effective, thorough and easy to follow. Laboratory personnel communicate well with
sample collectors and coordinate sample analyses effectively to manage workload and
holding times. Proficiency Testing (PT) samples for the 2014 proficiency testing calendar
year have not yet been analyzed. The laboratory is reminded that these results must be
submitted to this office directly from the vendor by September 30, 2014.
Contracted analyses are performed by Meritech, Inc. (Certification #165). TBL
Environmental Laboratory, Inc. (Certification #37) has been contracted in the past.
Current quality assurance policies for Field Laboratories and approved procedures for
the analysis of the facility's currently certified parameters were provided at the time of
the inspection.
III. FINDINGS, REQUIREMENTS, COMMENTS AND RECOMMENDATIONS:
Documentation
Comment: The laboratory did not have units of measure for pH or Temperature on their
benchsheets. The North Carolina Administrative Code, NCAC 02H .0805 (g) (1) states:

Data pertinent to each analysis must be maintained for five years. Certified Data must
consist of date collected, time collected, sample site, sample collector, and sample
analysis time. The field bench sheets must provide a space for the signature or initials of
the analyst, and proper units of measure for all analyses. Laboratory personnel
corrected the benchsheets during the inspection by adding units of measure. No further
response is necessary for this finding.
Comment: The lab was documenting analysis results on scrap paper. These scraps were
used to transfer the data to the lab's database. The database was used to generate a
printed benchsheet. The scrap papers are considered original records. Original records
are pertinent data. The printed benchsheets were being retained but the scrap papers
were not. The North Carolina Administrative Code, NCAC 02H 0805 (g) (1) states: Data
pertinent to each analysis must be maintained for five years. During the inspection,
laboratory personnel decided to ban the use of scrap paper and no longer print completed
benchsheets from the database. Now all data on the benchsheets will be hand written
instead of printed. The benchsheets are now the original records. They are being retained.
No further response is necessary for this finding.
IV. PAPER TRAIL INVESTIGATION:
The paper trail consisted of comparing field testing records and contract lab reports to
Discharge Monitoring Reports (DMRs) submitted to the North Carolina Division of Water
Resources. Data were reviewed for Cross Creek WRF (NPDES permit # NC0023957 for
October, November, and December, 2013. No transcription errors were detected. The
facility appears to be doing a good job of accurately transcribing data.
V. CONCLUSIONS:
All findings noted during the inspection were adequately addressed prior to the
completion of this report. The inspector would like to thank the staff for its assistance
during the inspection and data review process. No response is required.
