Box 118 Harmony, NC 28634 5605 September 29, 2011 Field Initial Chet Whiting Steve Lambert This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for the analysis of environmental samples. • The facility is neat and well organized and has all the equipment necessary to perform the analyses. Benchsheets are well designed, easy to follow and concise. Records are well organized and easy to retrieve. At the time of the inspection the laboratory was in the process of obtaining laboratory certification. A certificate was issued on October 27, 2011. - _ - • : - u k Comment: Sample site was not documented. 15A NCAC 2H .0805 (g) (1) states: Data pertinent to each analysis must be maintained for five years. Certified Data must consist of date collected, time collected, sample site, sample collector, and sample analysis time. The field bench sheets must provide a space for the signature or initials of the analyst, and proper units of measure for all analyses. Notification of acceptable corrective action (i.e. a revised benchsheet documenting sample site) was faxed to this office on October 4, 2011. No further response is necessary for this finding. Comment: A few overwritten data entries were observed, however the majority of corrections were performed properly. Quality Assurance Policies for Field Laboratories states: All documentation errors must be corrected by drawing a single line through the error so that the original entry remains legible. Entries shall not be obliterated by erasures or markings. Wite-Out®, correction tape or similar products designed to obliterate documentation are not to be used. Write the correction adjacent to the error. The correction must be initialed by the responsible individual and the date of change documented. All data and log entries must be written in indelible ink. Pencil entries are not acceptable. Notification of Page 2 #5605 Town of Harmony WWTP acceptable corrective action (i.e. a statement that correcting errors by overwriting will no longer be done) was received by e-mail on 10/19/2011. No further response is necessary for this finding. pH — Standard Methods, 20th Edition, 4500 H+ B Comment: The temperature sensing device on the pH meter had not been calibrated against an NIST certified thermometer. The Technical Assistance for Field Analysis of pH document states: All temperature sensing devices on meters must be calibrated against a NIST certified or NIST traceable thermometer annually (every 12 months) and proper corrections made and documented. The meter reading must be less than 10C from the NIST certified reading to be acceptable. Document the serial number and manufacturer of the NIST thermometer that was used in the comparison. Document any correction that applies (even if zero) on both the meter and on a separate sheet to be filed. Notification of acceptable corrective action (i.e. a copy of the temperature sensor calibration from Statesville Analytical Labs) was faxed to this office on October 4, 2011. No further response is necessary for this finding. Comment: There was no system in place to document Proficiency Testing (PT) samples in the same manner as routine environmental samples. Quality Assurance Policies for Field Laboratories states: The analysis of PT samples is designed to evaluate the entire process used to routinely repoii environmental analytical results; therefore, PT samples must be analyzed and the process documented in the same manner as environmental samples. Notification of acceptable corrective action (i.e. a statement that performance samples will be documented on bench sheets the same as samples) was received by e-mail on 10/19/2011. No further response is necessary for this finding. Comment: pH values were reported that exceed the method specified accuracy (0.1 S.U.) for this analysis. Standard Methods, 20th Edition, 4500 H+ B. (6) states: A variance of ± 0.1 pH unit represents the limit of accuracy under normal conditions, especially for measurement of water and poorly buffered solutions. For this reason, report pH values to the nearest 0.1 pH unit. Notification of acceptable corrective action (i.e. a statement that as of this time pH values would be reported to 0.1 S.U.) was received by e-mail on 10/19/2011. No further response is necessary for this finding. pH — Standard Methods, 20th Edition, 4500 O G Temperature — Standard Methods, 20th Edition, 2550 Comment: Sample collection and analysis times were not adequately documented. 15A NCAC 2H .0805 (g) (1) states: Data pertinent to each analysis must be maintained for five years. Certified Data must consist of date collected, time collected, sample site, sample collector, and sample analysis time. The field bench sheets must provide a space for the signature or initials of the analyst, and proper units of measure for all analyses. Notification of acceptable corrective action (i.e. a revised benchsheet documenting Temperature and DO samples were analyzed in situ) was faxed to this office on October 4, 2011. No further response is necessary for this finding. IV. PAPER TRAIL INVESTIGATION: The paper trail consisted of comparing field testing records and contract lab reports to Discharge Monitoring Reports (DMRs) submitted to the North Carolina Division of Water Quality. Data for NPDES# NC0087033 was reviewed for June and July, 2011. No transcription errors were detected. The facility appears to be doing a good job of accurately transcribing data.
