Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* The area of proposed stream restoration is situated along Petree Creek within the Sherwood Hills Townhome community. The project site is in a common area that occupies Forsyth County Tax Parcel 6806-97-7287 within the residential development. Due to the migration of Petree Creek, a portion of the streambank has been eroded to within 5-10 feet of an existing three-story townhouse at 3995A Valley Court. The lateral movement of the creek and resulting erosion is threatening to undermine the structure. 4b. Have Corps permits or DWR certifications been obtained for this project (including all prior phases) in the past? * Yes No Unknown 4f. List the total estimated acreage of all existing wetlands on the property: 0.0 4g. List the total estimated linear feet of all existing streams on the property: Perennial - 95 feet 4h. Explain the purpose of the proposed project: * Due to the migration of Petree Creek outside its original stream course, the streambank has been eroded to within 5-10 feet of a three-story townhouse at 3995A Valley Court. The lateral migration of the creek is threatening to undermine the structure. The proposed project will restore Petree Creek to its original stream course. A poured -in -place concrete retaining wall will be constructed between the creek and the vertical bank adjacent to the townhome. Engineered fill will be placed between the wall and the stream bank. 4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: * The project will involve the restoration of Petree Creek to a new trapezoidal channel within its original course along. A poured -in -place concrete retaining wall will then be constructed on the eastern stream bank. The area behind the wall will be backfilled and all disturbed areas revegetated. To complete this work, the following sequence will be followed: • Remove flagged trees along the eastern streambank within the project boundary. All materials shall be mulched on -site. • Install all fall protection and security fencing. • Install the wall protection blankets. • Construct the temporary access corridor. • Install erosion control elements. • Excavate the new trapezoidal channel. Place the excavated material in the existing stream channel. • Excavate the wall fin and footing. Place the excavated material in the existing stream channel and between the footing excavation and the eastern stream bank. Any excess water encountered during excavation shall be pumped to a point in the natural channel below the limits of the footing excavation. • A separate contractor will remove the existing sewer manhole and replace with a new manhole and piping. • Install concrete forms in the fin and footing. • Install rebar in the fin and footing. Pour the concrete for the fin and footing. The concrete will be poured using a concrete truck and boom in the parking lot. • Construct the retaining wall forms. Install the water control measures behind the wall forms. • Install the wall rebar in the forms. • Pour the concrete for the retaining walls. • Remove the wall forms. • Excavate, haul and place engineered fill between the retaining wall and the existing embankment. Seed and mulch the fill. • Remove all erosion control measures, fencing, and wall protection blankets. Restore any area of the grounds and yards that were damaged during construction. 5. Erosion control measures will be installed to limit impacts. The material excavated will be placed in the existing stream channel. During excavation for the wall fin and footing, any excess water encountered during excavation shall be pumped to a point in the natural channel below the limits of the footing excavation to limit erosion and sedimentation. 2.
