and turn left onto the entrance road for the Tar Heel trailer wash site. The trailer wash is at the end of
the entrance road anprox. 1/4 mile off of Purdie Hall rnnd

Describe receiving stream features and pertinent downstream uses: Goodman Swamp heads up near the
intersection of NC Hwy 20 and Purdie Hall road. Waterway is not well definedtypical of eastern NC
swamp and somewhat_ braided. Land uses downstream of the trailer wash adiacent to Goodmn
farms. Row crop production of corn/wheat/soybeans is also common.

If yes, please explain: The facility has not changed since the Iast permit renewal Adjacent properties and
the area in general around the trailer wash facility has not changed. No new residences or water supply
wells have been constructed on adiacent properties to the facility. Murphy -Brown LLC has permitted the
adjacent row crop fields (Tyson land NC-BL-20-3) for the land application of residual solids from the
trailer wash lagoons at Tar Heel and White Oak (WO0040332).

exceedance for nitrate over the last 10 years in MW-5 out of approx.. 150 samples taken Nitrate levels over
the last five years have averaged around 7 mvA with some concentrations at 8 m2/1 All monitoring wells
are the facility are inside the review boundary, between 50'-100' from the waste application area
Once MB removes solids from the facultative lagoon at the Tar Heel trailer wash in 2019/2020; nitrate
concentrations in the wastewater should decrease and minimize potential groundwater impacts in the
future.

Enclosed please find the Non -Discharge System Renewal Form: NDSR 09-18 and supporting documentation for
the Tarheel Sanitation Trailer Wash Facility.
The operations at this facility are remaining the same, however a change in the acres per spray application zone
is needed to reflect a reduction ,in acreage due to some sprinklers not being used to meet the required 150' property
line setback. Upon updating the site map, we discovered that several sprinklers were located within the setback. To date,
we have not been able to obtain property line setback waivers from the adjoining landowners and therefore are closing
these sprinklers off until the waivers can be obtained. The remaining acreage is enough to utilize the currently permitted
capacity of 12,410,000 gallons per year at 22 inches per acre per year. Please update the acreage per zone.
