ADDRESS: P.O. Box 1780
Elizabethtown, NC 28337
CERTIFICATE #: 5310
DATE OF INSPECTION: August 1, 2012

I. INTRODUCTION:
This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for
the analysis of environmental samples.
II. GENERAL COMMENTS:
The laboratory was clean and records were well organized. The system for traceability of consummables
is effective, thorough and easy to follow. This facility is only required to monitor when discharging,
approximately once a year. As a result, there was no current bench or contract environmental data for
review. Proficiency Testing sample data for the 2012 calendar year was reviewed.
The laboratory was given a packet containing North Carolina Laboratory Certification quality control
requirements and policies during the inspection.
Contracted analyses are performed by Microbac Laboratories, Inc. (Certification #11).
III. FINDINGS, REQUIREMENTS, COMMENTS AND RECOMMENDATIONS:
Dissolved Oxygen – Standard Methods, 18th Edition, 4500 O G
Comment: The laboratory was not documenting the meter calibration time on the benchsheet. The
Approved Procedure for Field Analysis of Dissolved Oxygen document states: The following criteria must be
documented whenever sample analysis is performed. 5. Meter calibration and time(s) of calibration.
Notification of acceptable corrective action (i.e., an updated benchsheet which includes a space to
document the calibration time) was received by email on 08/08/12. No further response is necessary for
this finding.
IV. PAPER TRAIL INVESTIGATION:
No paper trail performed.

report. The inspector would like to thank the staff for its assistance during the inspection and data
review process. No response is required.
