Good afternoon Dave, Eric, and Stephen,
Happy New Year! I am emailing in regard to the proposed Big Mill Farm Road Widening and Extension project (TIP# U-
5760) Individual Permit Modification in Kernersville, Forsyth County, NC. We would like to schedule a pre-application
meeting to briefly review the project history and discuss the design modifications that have resulted in changes to the
previously permitted impacts (SAW-2016-01337) in order to streamline the permitting process.

The NCDOT-Division 9 proposes to widen and extend Big Mill Farm Road and widen Hopkins Road from I-40 Business/US
421 to West Mountain Street with a new partial cloverleaf interchange at I-40 Business/US 421. The Section 404/401
Individual Permit was approved by the USACE on August 10, 2020 and by the NCDWR on July 13,2020 (see attached IP
Approval Letters). Since the 404/401 Approvals, the NCDOT Geotech and Construction Units recommended changes to
the proposed retaining walls along the western side of Big Mill Farm Road, within loop D of the partial cloverleaf
interchange, and within loop B of the partial cloverleaf interchange. The design changes would shorten the overall wall
heights and revise the associated the slope and drainage and have been recommended due to constructability,
practicability, and maintenance concerns with the previously proposed retaining walls. Following discussions between
utilities staff, drainage staff, and the Division, a constructed riffle was added on the downstream side of a proposed two-
barrel box culvert, north of Hopkins Road at the previously permitted Impact Site 4A to better accommodate the existing
sewer line and provide protection/stabilization to prevent scouring or erosion downstream of the proposed culvert.
Overall, these plan modifications will only result in updated impact amounts/type at two previously permitted impact
sites.
At the pre-application meeting, we will summarize the modifications to the proposed impacts, discuss the minimization
measures that were previously applied, and review the reasoning behind the modifications to the previously permitted
project design. We will share a draft set of updated permit drawings at the meeting to highlight the changes to the
previously permitted impact amounts and/or type as a result of the design modifications.
