1. Permanent and temporary stream impacts are listed as Sites S1 through S11 and are
included in the PCN and this attached sheet. Additional potential jurisdictional streams
assessed to be ephemeral drainage ditches (Sites S12 through S45) are more numerous than
available space on the PCN and are included on this sheet.
2. Activities are anticipated to be authorized by NWP 3 Maintenance; therefore, only new
impacts related to necessary structure and fill improvements are included.
