the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in'the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment

to accommodate any proposed/future growth while plans for upgrades are implemented.
Therefore:

This application is for an extension of the gravity sewer system of Johnston County to serve Fieldview
Village Estates. This extension includes 5,9031f of new 8" gravity sewer and will connect to an existing
Johnston County gravity sewer system permit number WQ0042378. The sewer system will serve 51
single family homes in this subdivision. which is the basis for flow for this project.

We're reviewing the referenced sewer extension application and have the following question:
1. Item IV of the application (FTA 06-21) states that the wastewater will be treated at the Town of Benson's WWTP
(NC0020389). Two Flow -Tracking forms (FTSE 10-18) were submitted, one for Four Oaks' downstream pump
stations, and one for the Central Johnston WWTP, signed by Chandra Farmer. But there isn't an FTSE for
Benson's WWTP. Will this flow be treated at the Central Johnston WWTP, or Benson's WWTP? IF it's Benson,
we will need an USE from them.
Sorry, this project's flow is not going to Benson. That was a mistake. Sheet 1 of 5 has been correct and is
attached.
2. The FTSE for Four Oaks shows that the Northside PS is 25,700 GPD over capacity.
a. Is this before or after the addition of 18,360 GPD included in this current application? As per
conversation with the Town and their Consultant, the 18,360 GPD is included in the current application.
b. The addendum states that Four Oaks has received money to upgrade the Northside Pump Station, but it
appears that no permit has been issued for upgrades, and possibly no plans have currently been
developed to upgrade the pump station? Is that the case? And is the money to upgrade the Northside
Pump Station ARPA funded? As per conversation with the Town and their Consultant, the Town has
contracted with The Wooten Company to upgrade various stations in Four Oaks including the Northside
Pump Station.
