Dear Mr. LaClair:
Enclosed is a report for the inspection performed on April 12, 2016 by Tonja Springer. Since the
Finding(s) cited during the inspection were all corrected prior to the completion of the enclosed report, a
response is not required. The staff is commended for taking the initiative in correcting the Findings in
such a timely manner. For Certification maintenance, your laboratory must continue to carry out the
requirements set forth in 15A NCAC 2H .0800.

This laboratory was inspected by a representative of the North Carolina Wastewater/Groundwater
Laboratory Certification (NC WW/GW LC) program to verify its compliance with the requirements
of 15A NCAC 2H .0800 for the analysis of environmental samples.
II. GENERAL COMMENTS:
The facility has all the equipment necessary to perform the analyses. Staff were forthcoming and
helpful in answering our questions.
Temperature was removed from the facility's Certified Parameter Listing effective May 2, 2016. It
was not needed since the parameter is no longer included in the City of Roxboro's WWTP
(NC0021024) pretreatment agreement with Eaton and Eaton's Stormwater permit (NC0003379),
is no longer active.
Proficiency Testing (PT) samples for the 2016 proficiency testing calendar year have been
analyzed but the final report has not yet been received.
Contracted analyses are performed by Meritech, Inc. (Certification #165).
Current Quality Assurance Policies for Field Laboratories and a copy of the Approved Procedure
for the Analysis of pH were provided at the time of the inspection.
Ill. FINDINGS, REQUIREMENTS, COMMENTS AND RECOMMENDATIONS:
Documentation
Comment: The laboratory needs to increase the documentation of purchased materials. The
date opened, date put into use is not being documented. The Quality Assurance Policies for
Field Laboratories states: All chemicals, reagents, standards and consumables used by the
laboratory must have the following information documented: Date Received, Date Opened (in
use), Vendor, Lot Number, and Expiration Date. This information as well as the vendor and/or
manufacturer, lot number, and expiration date must be retained for chemicals, reagents,
standards and consumables used for a period of five years. Consumable materials such as pH

buffers and lots of pre -made standards are included in this requirement. Notification of
acceptable corrective action (i.e., updated pH benchsheet which included date opened and date
put into use with an implementation date of 5/2/2016) was received by email on 5/5/2016. No
further response is necessary for this Finding.
Proficiency Testing
Comment: The laboratory is not analyzing Proficiency Testing (PT) samples in the same manner
as compliance samples. A known sample is analyzed along with the PT samples. Since this is not
performed with all environmental samples, it is considered additional quality control. However,
known samples are recommended when analyzing remedial PT samples as part of the
troubleshooting and corrective action process. The Proficiency Testing Requirements, February
20, 2012, Revision 1.2. states: All PT samples are to be analyzed and the results reported in a
manner consistent with the routine analysis and reporting requirements of compliance samples
and any other samples analyzed according to the requirements of 15A NCAC 2H .0800.
Notification of acceptable corrective action (i.e., PT samples procedure that stated, "PT samples
are to be handled and analyzed in the same manner as environmental regulatory samples
collected by Eaton Roxboro personnel," with an implementation date of 2017 PT samples) was
received by email on 4/25/2016. No further response is necessary for this Finding.
Comment: The laboratory was not documenting PT sample analyses in the same manner as
compliance samples. The PT sample was only documented on the vendor reporting form and not
the benchsheet. The Proficiency Testing Requirements, February 20, 2012, Revision 1.2
document states: All PT sample analyses must be recorded in the daily analysis records as for
any environmental sample. This serves as the permanent laboratory record. Notification of
acceptable corrective action (i.e., a benchsheet showing the documentation of the analysis of the
2016 PT sample with an implemented date of 4/25/2016) was received by email on 4/25/2016. No
further response is necessary for this Finding.
IV. PAPER TRAIL INVESTIGATION:
No paper trail was performed.
V. CONCLUSIONS:
All Findings noted during the inspection were adequately addressed prior to the completion of this
report. The inspector would like to thank the staff for its assistance during the inspection and data
review process. No response is required.
