To Whom It May Concern:
The sewer extension for the above -mentioned project has been submitted for approval and we received comments on
October 25, 2022. The comments have been addressed as follows:

1) Items IV & V were left blank. Please complete these Sections.
Response: These sections have been revised,
2) The narrative states this permit will include 121 lots, but Section VI.7 show 545 bedrooms. Please clarify the total
number of homes and bedrooms..
Response: The narrative has been revised to include the 121 lots.
3) Section VI.6 is checked 'No' for a €tow reduction, and that's correct, Middlesex does not have an approved flow
reduction. However, Section VU uses 80 gal/bedroom, which is not the design flow from the 2T Rules. 120
GPDlbedroom should be used (min. of 240 per house). After specifying the number of homes/bedrooms, please
recalculate the flow using the correct design flow.
Response: The flows have been corrected to reflect the 120 GPD1bedroom as shown In the 2T Rules.
4) Item IX.4 is checked 'No" for being in a State -buffered basin. However, Middlesex is in the Neuse River basin and
subject to the Neuse Buffer Rules. This Item should be checked "Yes', and it should be confirmed that the project
meets the requirements.
Response: Item IX.4 has been revised.
5) Item IX.7 is checked 'No' for high -priority lines. Since there is almost 8,000 LF of gravity line, please confirm there
are no aerial crossings or sewedines positioned parallel to stream banks that are subject to erosion

Response: The sewer is being bored under the stream so there are no aerial crossings and all sewer lines are
outside the buffers of the stream.
6) The Flow Tracking Sheet (FTSE 10-18) that was included appears to include Middlesex's downstream pump
stations, and was signed by a representative of Middlesex. We'll also need a second FTSE form that includes

Please find the enclosed application, plans and supporting documentation for the Middleton Farms Subdivision for
your review and approval.
This project proposes the addition of 121 residential single family units. The project will be completed in 2 phases. At
this time, we are seeking to provide gravity sewer service for a total of 121 lots within phases 1 and 2 of the project.
The Town of Middlesex does not wish to restrict development in the area so they worked with us and the developer to
reach an acceptable daily flow rate that would allow the Town of Middlesex to provide an adequate level of service
while still allowing growth in the area and not further diminishing the towns sewer allocation from City of Raleigh.
Town of Middlesex was consulted and agreed to the number and types of units to reach an average daily flow that
could be accommodated by the current allocated flow. The flows, number of units and total daily flow are included on
the application form as required.

Mr. Speight & Mr. Lewis,
We are reviewing the referenced sewer extension and have the following
comments/questions:

1. Items IV & V were left blank. Please complete these Sections.
2. The narrative states this permit will include 121 lots, but Section V1.7 show 545
bedrooms. Please clarify the total number of homes and bedrooms.
3. Section V1.6 is checked "No" for a flow reduction, and that's correct, Middlsex
does not have an approved flow reduction. However, Section VI.7 uses 80
gal/bedroom, which is not the design flow from the 2T Rules. 120
GPD/bedroom should be used (min. of 240 per house). After specifying the
number of homes/bedrooms, please recalculate the flow using the correct
design flow.
4. Item IX.4 is checked "No" for being in a State -buffered basin. However,
Middlesex is in the Neuse River basin and subject to the Neuse Buffer
Rules. This Item should be checked "Yes", and it should be confirmed that the
project meets the requirements.
5. Item IX.7 is checked "No" for high -priority lines. Since there is almost 8,000 LF
of gravity line, please confirm there are no aerial crossings or sewerlines
positioned parallel to stream banks that are subject to erosion.
6. The Flow Tracking Sheet (FTSE 10-18) that was included appears to include
Middlesex's downstream pump stations, and was signed by a representative of
Middlesex. We'll also need a second FTSE form that includes the values for the
downstream treatment plant (Item 1), and signed by a representative of the
downstream treatment plant (Raleigh). It should also include any downstream
pump stations in Raleigh's collection system. It should show the correct
requested flow on 1.f (after Item 3 above is addressed)
