Division of Water Resources' Central and Regional staff has reviewed the application package
received July 25, 2022. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on October 17, 2022.

C. Wastewater Irrigation System —Renewal (FORM: WWIS-R 02-21) Application:
Applicant Name: The applicant's name in Section I, Item 1 was given as "Falls Lake SRA —
Rollingview" on the Wastewater Irrigation System Renewal Form, but the owner of this facility
and Permittee is currently the 'North Carolina Department of Natural and Cultural Resources".
"Falls Lake SRA — Rollingview" is the name of the facility. Please clarify which organization is
the Permittee who is applying for the renewal and ensure that this is the same as the organization
which will own the facility. An updated application shall be provided with the corrected Applicant's
name, if needed.
2. Section V — Operator in Responsible Charge (ORC) Information: The application lists
Christopher McGee and Vincent Shea as the ORCs for this facility. Both are currently designated
as a backup operator. In accordance with 15A NCAC 08G .0201, water pollution control systems
shall designate one ORC and one or more Back-up ORCs with valid certificates. Please contact the
Wastewater Operator Certification ._r�oup in order to designate a primary ORC.
Section IX — Irrigation Fields: The application has combined Fields 1 and 2, giving both GPS
coordinates of (36.007750,-78.728924). This location is to the west of Baptist Road. However, the
current permit lists Field 1 at GPS coordinates (36.007500,-78.727222), which is to the east of
Baptist Road, and places Field 2 at GPS coordinates (36.4575,-78.727222), which is in Oxford,
NC. Section IX shall be updated to list Field 1 and Field 2 separately and clarify where these fields
are located.
4. Attachment L —Setback Waivers: The applicant selected "no" when asked whether the existing
permit includes setback waivers. However, the existing permit does include setback waivers.
Pursuant to 15A NCAC 02t .0505(d), please provide copies of all setback waivers that have been
written, notarized, signed by all parties involved, and recorded with the county Register of Deeds.

1. The current permit indicates that this facility includes approximately 3.55 miles of sewer line. In
accordance with 15A NCAC 02T .0403(a)(2) and Condition IL3.a. of your previous permit dated
January 23, 2017, a map of the sewer system shall be actively maintained. Please provide a map
clearly depicting this sewer line and its relation to the treatment units and/or irrigation fields.
