Tar-Pamlico Buffer Authorization (DWQ Project No. 20140380).

Subject:
Modification request due to additional buffer impacts associated with
the proposed widening of SR 1211 (West River Road) in Franklin County;
WBS No. 49011
W-5509. .

The North Carolina Department of Transportation (NCDOT) has W-5509 under contract and
the culvert wing extension work is scheduled to begin soon. Recent review of the project limits
revealed an access issue for safely getting equipment next to the culvert on the SE quad.
Therefore the contractor has requested additional easement beyond that presented in the
April 11, 2014 application.

Therefore Division 5 is providing this letter to report these extra proposed impacts and to
request a buffer authorization modification for the additional buffer impacts totaling 250 SF
(150 SF in BZ1 and 100 SF in BZ2), see attached drawing. Note the adjacent off site wetland
