Ms. Smith,
We have attached a letter requesting a change to DWR Permit # 14 -0908. We have also attached the permit and a map
showing the new buffer impact. The change in the impact is due to a realignment of a sanitary sewer, a property owner
did not want the impact of the original alignment across their property. We have adjusted the alignment and are
requesting a revised permit to show these changes. Let me know if you have any questions.

Dear Ms. Smith:
The project titled "Swann Street Sanitary Sewer Replacement and Relocation' DWR # 14 -0908 (attached) consists
of rerouting a portion of the existing sanitary sewer line away from an Unnamed Tributary to Crabtree Creek to
prevent future impacts to the sewer due to natural stream movements and replacing in -place a portion of sanitary
sewer to maintain gravity flow. This activity will occur in a sanitary sewer easement that has not been maintained in
the last 10 years. The following buffer impacts have changed with a new realignment of the sewer and map i5
attached.

The total temporary buffer impacts are 9,999 and 3,778 sq. ft. for zone 1 and 2, respectively. The project will also
result in 40 linear feet of temporary stream impacts. The buffer disturbance is a result of easement clearing
necessary to access the repair location.
