This project meets the documentation requirements and approval procedures under NEPA, as defined by FHWA, for Low Impact Bridge Replacements. This project meets the standards of NCDOT's Minimum Criteria Rules. Date: 1/21/15 Applicant Name: NC DEPARTMENT OF TRANSPORTATION Applicant Address: PO BOX 47 1710 E. MARION STREET SHEIBY. NC 28151 Primary Contact for Project: TRISH BEAM Phone No.: (704) 480-9044 Is this an after-the-fact application: Yes X No River Basin: CATAWBA Stream Classification: C Regulatary Authorization Options for this Activity Federal: USACE Nationwide General Permit 3— Maintenance State: General Water Quaiity Certification #3687 Locai: None The proposed bridge will be placed on its xisting alignment. The new bridge structure will be a 3-span, 1@35', 1@55', 1@35' — 21" deep cored slab bridge (OAl = 125' long by 36' wide). There are no wetlands present within the proposed limits of construction. The placement of riprap bank stabilization under the proposed bridge, as weli as riprap slope protection for (1) existing and (1) proposed ditch, results in 63 linear feet of permanent stream impacts. An off-site detour will be utilized during"construction. This bridge is following the Low Impact Bridge Projess established by FHWA, NCDOT, and Regulatory Agencies. ...,., / .
