Dear Mr. Watson:
On December 13, 2022, staff of the North Carolina Division of Water Resources' Winston-Salem
Regional Office (DWR) performed a compliance inspection of the single-family residence
wastewater irrigation system located at the subject property. This compliance inspection was
conducted by DWR staff person Mr. Jim Gonsiewski. While inspection of the wastewater
irrigation system generally reflects compliance with Permit No. WQ0006457, the following items
of concern require your attention and action:
1) You stated that you check the tablets in the chlorinator and the components of the system
monthly and have kept records of these inspections. Permit Condition III. (12) lists specific
inspection frequencies (weekly for the chlorinator and monthly for the storage, pump,
alarm systems, spray system). Permit Condition V. (2) states that a record of these
inspections should be maintained for five years. Additionally, the current operational and
maintenance agreement lists these requirements.
Please begin inspecting your chlorinator weekly and continue to maintain an
inspection log.
2) Based on our records, the annual permit fee of $60 has not been paid for 2021 or 2022.
Your permit expires on March 31, 2023.
Please pay these fees as soon as possible so your permit can be renewed on time.

Failure to abide by the conditions contained in your permit may subject the permittee to
enforcement action by DWR in accordance with North Carolina General Statute 143-215.6A to
143-215.6C and/or revocation of this permit.

Inspection Summary:
On December 13, 2022, staff of the North Carolina Division of Water Resources' Winston-Salem Regional Office (DWR)
performed a compliance inspection of the single-family residence wastewater irrigation system located at the subject
property. This compliance inspection was conducted by DWR staff person Mr. Jim Gonsiewski. While inspection of the
wastewater irrigation system generally reflects compliance with Permit No. WQ0006457, the following items of concern
require action:
1) The permittee stated that he checked the tablets in the chlorinator and the components of the system monthly and has
kept records of these inspections. Permit Condition 111.(12) lists specific inspection frequencies (weekly for the chlorinator
and monthly for the storage, pump, alarm systems, spray system).Permit Codition V.(2) states that a record of these
inspections should be maintained for five years. Additionally, the current operational agreement lists these requirements.
It was recommended that Mr. Watson begin inspecting the chlorinator weekly and continue to maintain an inspection log.
2)Based on DWR records, the annual permit fee of $60 has not been paid for 2021 or 2022. The permit expires on March
31, 2023.
It was recommended that the permittee pay these fees as soon as possible so the permit can be renewed on time.

Arrived on site at 03:20 PM, 12/13/2022. Met with the owner, Randall Watson. Walke
through the system. Everything appeared to be in good operational order. According
to Mr. Watson, they do not have any problems with the system. He and his wife and
his two children are the only individuals living at the residence. The correct chlorine
tablets are being used. The alarm for the system is located at the system pump
outside the house. The system was tested and works. Mr. Watson stated that the
alarm can easily be heard inside the house.
The septic tank (1,000 gallon, baffled) is located on the east side of the house. Two
concrete access hatches were visible. The line from the tank runs southeast to the
396 square foot sand filter. The line from this filter continues to run southeast
towards two chlorinator tubes in a tree line. The line from the tubes runs northeast
towards a chlorine contact tank and a pump tank. The line from this
pump tank runs southeast to the 0.15 acre spray field located inside the property line.
The spray field is surrounded by a 2 wire fence”and has four spray heads. The water
supply well for the residence is located on the Ikafs411461f3tWe property near Isley
School Road. Walked around the spray field. No runoff or ponding was noted in the
field. Mr. Watson turned on the pump for the system. All four spriklers were
operational. No problems with the spray pattern. Mr. Watson also stated that on
11/22/2022, in conjunction with the pumping of the septic tank, the system was
inspected and serviced by Rowland Environmental.
Mr. Watson said he checks the chlorinator tubes and the components of the
system monthly. He has a record of these inspections. Reminded him that the
permit requires the chlorinator should be checked weekly.
