As of March 04, 2014, Excelsior Packaging Group of Yonkers, NY, and holder of NPDES permit No.
NC0000108, ceased to own /operate its Brevard, North Carolina facility. The facility, property, and
equipment were purchased effective March 4t' by Sigma Plastics Group of Lyndhurst, NJ and
incorporated under the state of North Carolina as New Excelsior, Inc. All operations, and the
wastewater discharge, have remained the same. The facility is continuing to operate under the
parameters of the permit without change and with the same ORC, James and James Environmental.
Enclosed are two copies of the Permit Name /Ownership Change form, as well as two copies of the
Articles of Incorporation for New Excelsior, Inc. Jim Kelley has remained as General Manager and is
signing the form as responsible official for the permit under New Excelsior, Inc.
