Notes about the attachments:
The uploaded application forms are *not' signed because the State rejected the previous copies when they were
submitted - even though the "certification" below demands that I agree to the validity of electronic signature (while DEQ
rejects non -ink signatures on the forms I submit). Therefore, no copies were made of the forms, this time. Hence, the
original/wet signed versions were submitted, and the uploaded copies are not signed.
