Our records show that you have not submitted a DMR for the following month(s) [see table below]. (2.) of the NPDES permit, as well as 15A NCAC 2B .506(a), which requires the submittal of Discharge Monitoring Reports no later than 30 calendar days after the end of the reporting period for which the report is made. Failure to submit reports as required will subject the violator to the assessment of a civil penalty of up to $25,000 per violation. To prevent further action, please submit the reports within fifteen (15) days of receipt of this notice. If you have documents proving that this record is in error, please forward them to our office. If this fadlity is inactive or not constructed, you may submit a written request to suspend monitoring. The request should include a begin date and an end date for the period of monitoring.
