Division personnel have reviewed and approved your application for renewal of the subject permit. Accordingly,
we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the
U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
This final permit includes no significant changes from the draft permit sent to you on July 18, 2012. The name of
the corporation has been corrected on the map, as requested in your August 10, 2012 e-mail message.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this
letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to
obtain other permits which may be required by the Division of Water Quality or permits required by the Division
of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that
may be required.

This permit shall become effective February 1, 2013.
This permit and authorization to discharge shall expire at midnight on September 30, 2017.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
Carolina Water Service, Inc. of North Carolina

1. Continue to operate an existing 0.040 MGD package plant and a 0.100 MGD package plant,
operating in parallel, each with:
• Aeration basins

The facility is located north of Shulls Mill at the Hound Ears WWTP, at 1624 Dogwood Road, in
Watauga County.
2. Discharge from said treatment works at the location specified on the attached map into the Watauga
River, classified B-Trout HQW waters in the Watauga River Basin.

There shall be no discharge of floating solids or visible foam in other than trace amounts

means of disinfection is required. Total Residual Chlorine monitoring requirements and limits are
applicable if chlorine compounds are used for disinfection.

A. (2.) TEMPORARY MEANS OF DISINFECTION
In the event that the wastewater treatment plant's ultraviolet (UV) disinfection system should fail,
underperform, or otherwise be removed from effective service, the permittee or his agent should
immediately inform the Surface Water Protection Section staff of the Winston-Salem Regional Office
of this development and discuss temporary, alternate means for disinfection of the effluent.
If chlorine compounds are used as temporary means of disinfection, the total residual chlorine (TRC)
concentration of the effluent must be monitored on a daily basis while this method of disinfection is in
use and values must be reported on the discharge monitoring report. TRC in the effluent may not
exceed 28 µg/L; therefore, dechlorination methods may also be necessary to ensure protection of water
quality in the receiving stream.
In the event that TRC monitoring should occur, the Division shall consider all effluent TRC values
reported below 50 µg/L to be in compliance with the permit. However, the Permittee shall continue to
record and submit all values reported by a North Carolina certified laboratory (including field
certified), even if these values fall below 50 µg/L.
Use of a disinfection system other than the permitted UV system during circumstances not deemed as
emergency and/or temporary (replacement of lamps, lamp cleaning or maintenance, etc.) will be
considered a violation of the terms of this permit.

The North Carolina Environmental Management
Commission proposes to issue a NPDES wastewa-

will be accepted until 30 days after the publish

cis Elementary School WWTP in Watauga County;

Dutch Creek in the Watauga River basin.
The Watauga County Board of Education request-
ed renewal of permit NC0066991 for the Bethel Ele-
mentary School WWTP in Watauga County; this
permitted discharge is treated wastewater to
Beaverdam Creek in the Watauga River basin.
Water Quality Utilities, Inc. requested renewal of
permit NC0050610 for The Ponds WWTP in Watau-
ga County; this permitted discharge is treated do-
mestic wastewater to the Watauga River in the
Watauga River basin.
Seven Devils Resort WWTP, NCSR 1151, Seven
Devils, Watauga County, applied to renew permit
NC0035149, discharging to a UT of the Watauga
River, Watauga Basin.
Water Quality Services, Inc. requested renewal of
permit NC0032212 for the Yonahlossee WWTP in
Watauga County; this permitted discharge is
treated domestic wastewater to Lance Creek in
the Watauga River basin.
Carolina Water Service, Inc. of North Carolina re-
quested renewal of permit NC0032123 for the
Hound Ears WWTP; this permitted discharge is
treated domestic wastewater into the Watauga
River in the Watauga River Basin.
Town of Beech Mountain applied to renew NPDES
permit NC0088099 for the Buckeye Lake WTP, 1400
Pine Ridge Road, Watauga County, discharging
treated wastewater to Buckeye Creek, Watauge
River Basin.
Cove Creek Preservation and Development, Inc.
requested renewal of permit NC0067008 for the
Old Cove Creek School WWTP in Watauga County;
this permitted discharge is treated wastewater to
Cove Creek in the Watauga River basin.

We are in receipt of your July 18, 2012 letter concerning the above referenced matter and after review, we only found
one very minor issue.
On the facility location map, our official name is slightly incorrect. The "Inc." should be after "Service". In all other
locations it was correct. Not a big deal but thought it should be pointed out.
