Dear Mr. Lashua:
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as
subsequently amended).
This final permit includes the following changes from the draft permit sent to you on July 18, 2007:
• Corrections and modifications have been made to the permit based on the comments you provided in
your July 26, 2007 letter.
• The explicit monitoring requirement and associated effluent limitation for total residual chlorine
(TRC) have been removed from the permit. A condition has been added describing procedures the
facility must follow in the event that its UV system should fail and chlorine compounds are used as
temporary means of disinfection.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or
Local governmental permit that may be required.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management
Commission, and the Federal Water Pollution Control Act, as amended,

Hound Ears WWTP
1624 Dogwood Road
North of Shulls Mill
Watauga County
to receiving waters designated as Watauga River in the Watauga River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts
I, II, III and IV hereof.
This permit shall become effective November 1, 2007.
This permit and authorization to discharge shall expire at midnight on September 30, 2012.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
Carolina Water Service, Inc. of North Carolina is hereby authorized to:
1. Continue to operate an existing 0.040 MGD package plant and a 0.100 MGD package plant with
the following additional components:
• Sludge basin
• Ultraviolet disinfection
The facility is located north of Shulls Mill at the Hound Ears WWTP at 1624 Dogwood Road in
Watauga County.
2. Discharge from said treatment works at the location specified on the attached map into the Watauga
River, classified B-Trout HQW waters in the Watauga River Basin.

A. (2.) TEMPORARY MEANS OF DISINFECTION
In the event that the wastewater treatment plant's ultraviolet (UV) disinfection system should fail,
underperform, or otherwise be removed from effective service, the permittee or his agent should
immediately inform the Surface Water Protection Section staff of the Winston-Salem Regional Office
of this development and discuss temporary, alternate means for disinfection of the effluent.
If chlorine compounds are used as temporary means of disinfection, the total residual chlorine (TRC)
concentration of the effluent must be monitored on a daily basis while this method of disinfection is in
use and values must be reported on the discharge monitoring report. TRC in the effluent may not
exceed 28 µg/L; therefore, dechlorination methods may also be necessary to ensure protection of water
quality in the receiving stream.
Use of a disinfection system other than the permitted UV system during circumstances not deemed as
emergency and/or temporary (replacement of lamps, lamp cleaning or maintenance, etc.) will be
considered a violation of the terms of this permit.

I am attaching a suggested redline for your review. Sorry for the delay and thank
you for allowing the opportunity to comment.
I understand why you wish to change from the draft to this new wording so that it
is clear taking the UV off line is a permit violation whereas before, it implied
that you could take it off line as long as you monitored TRC?
"Chlorine" is used so loosely and probably should be further expanded. The only
true "chlorine" is chlorine gas which is much less common than it used to be. It
is more common to use sodium hypochlorite (bleach) or calcium hypochlorite
(tablets) etc. Not sure if this is needed.
It would not be possible to meet TRC of that low without dechlorination so I added
some words to that affect.
There are brief periods of time when the UV has to be taken out of service for
maintenance such as lamp cleaning and/or replacement. We would like to try and
make routine maintenance issues less of a concern but there are situations when
this will be necessary.
Lastly - since the facility is not permitted to use other forms of chlorination and
dechlorination, is this going to cause a problem that we are going to have to
engineer, get ATC and permit for the "back up" systems we will need in the event
the UV does not work? Seems like someone will immediately chastise us for using
other "non -permitted" mechanisms even in an emergency?.njdlab2! ei

1) The WWTP address is 1624 Dogwood Road, Blowing Rock, NC 28605. It may be
best to use this address if you feel it is appropriate.
2) The proposed daily maximum for NH3 as N has a typo error for units. It is shown as
"ug/L" and should be "mg/L".

continued discharge of treated sanitary and Indus-
trial wastewater to the Yadkin River in the Yadkin
Pee -Dee River Basin. There will be no increase in
pollutant loadings. No parameters are currently
water quality llmlted, however, this discharge may

Yadkin River Basin.
