Paul Wilms Director A letter of request for Authorization to Construct was received May 24, 1988, by the Division and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization .is hereby granted for the construction of additional facilities for the Town of. Forest City Wastewater Treatment Plant to increase the capacity from 2.25 MGt) to 4.25 MGD. The expansion includes the construction of a mechanical bar screen, a combination grit and grease separator tank (62 ft by 7.25 ft: by 13,25 ft. for grit and 62 ft. by 6.6 ft. by 13.25 ft. for grease) with divider baffles, a traveling bridge for each unit, n 75 GPM grit pump, and di.iai 130 CFM blowers, a 60 foot diameter primary clarifier with sludge collector, a 68 foot diameter trickling filter with dual 3700 GPM pumps, dual 14 foot by 54 foot: aeration. basins (converted from clarifier) with sp.l_i.tter box, floating aerators, aeration headers, dual 1100 GPM jet aeration pumps, two (2) 75 foot diameter secondary clarifiers with splitter box and sludge collectors, modifications to the existing pump station to include new check and grate valves and aeration basin feed pumps, modifications to the existing recirculation pump station to include dual 1500 GPM recirculation pumps and dual 260 GPM waste sludge pumps, the modification of the, existing chlorine contact tank to include new baffles and fine bubble diffusers, the addition. of. a 1.36,000 gallon chlorine contact tank, the conversion of the existing secondary clarifier Lo a sludge holding tank, a 55 foot diameter digester with a. 4000 GPM jet aeration system pump, a 3000 GPM jet aeration system pump for the existing digester, a 750 GPM sludge pump, dual. 390 GPM scum pumps, a 260 GPM thickened sludge pump, a sludge dewatering mechanism with a dewatering tank and screw, overflow weir, and discharge spout, two new post aeration tanks with fine bubble aeration, a belt sludge thickener, and all other appurtenances to serve the Town of Forest City. Raleigh, North Carolina 27611.7687 Telephone 919-733-701 c An Eoual Opportunity Affirrnarive Action Employer This Authorization to Construct is issued in accordance with Part III, paragraph B of NPDES Permit No. NC0025984 issued October 8, 1987, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NCOO25984. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph C of the referenced permit. The sludge generated from these treatment facilities must be disposed of in accordance with General Statute 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. The Asheville Regional Office, phone no. 704/663-1699 shall be notified at least twenty-four (24) hours in advance of operation of these facilities. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. In the event the facilities fail to perform satisfactorily in meeting its NPDES permit effluent limits, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional wastewater treatment and disposal facilities. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been .i.nsLalled in accordance with the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687. A copy of the approved plans and specifications shall be maintained on file by the permittee for a minimum of five years from the date of issuance.
