Please correct the information for the listed fields to represent the proposed Field IDs WB-001 through WB-008 and ensure the combined acreage and predominant soil type are correct. Please also correct the acreage of land application sites currently certified which should match the most recent permit issuance (i.e., 1467.66). D. Application (Form: RLAP 06-16): 1. Item 3 — The ORC and Back -Up ORCs listed here do not match what we have on record. Currently, Rebecca G. Miller is listed as the ORC and Andrew C. Edwards is listed as the Back -Up ORC. Ms. Miller's operator certification is also invalid. See Section E.1. 3. A review of our records shows that Dwan Bell has not been designated as a signature authority for this permit. Per 15 NCAC 02T .0106, the current City Manager, Montre Freeman, must sign this application, or Mr. Freeman must complete a Signature Authority Designation Letter for Mr. Bell to sign. Please either have Mr. Freeman sign Page 5 and resubmit or provide a Signature Authority Delegation Letter for Mr. Bell. E. Water Pollution Control System Operator Designation (Form: WPCSOCC): 1. H. Residuals Source Certification for Elizabeth City WTP (Form: RSC 06-16): 1. No comment. I. Land Owner Agreement (Form: LOA 06-16): 1. Please correct the Field IDs listed in the table on Page 1 to represent the proposed Field IDs WB- 001 through WB-008. 2. If a Signature Authority Designation Letter is not submitted for Mr. Bell, please have Mr. Freeman sign Page 3 of the LOA and resubmit as per 15 NCAC 02T .0106. I. Site and Setback Maps: Please resubmit setback maps of WB-001 through WB-008 with the most recent aerial imagery available and following the items listed under 15 NCAC 02T .1104(c)(1) and Map Guidance for Residuals Land Application Fields, which has been attached. There are structures shown on aerial imagery within Field WB-008 that are not accounted for with the field area in the setback maps submitted. Please double-check that the latest dwellings, structures, wells, and surface drainage features are shown and included in the setback geometry. Are the existing ponds shown within Field WB-008 being used for stormwater management? J. Soil Samples: 1. No comment. K. Agronomist Evaluation: 1. No comment. L. Operation & Maintenance Plan: 1.
