Please find attached four copies of the Pre - Construction Notification Application for the above
referenced site The site contains recently timbered land The site is being proposed for Phases
I and II of a single family residential development
In order to develop the site, a road must be constructed that will cross a stream. The road will
permanently impact 116 45 linear feet of stream channel (rip rap and culvert) Additionally, the
pump around for the road construction area and sewer line connections will temporarily impact
96 linear feet of stream channel Mitigation is not proposed
I am requesting that a 401 Water Quality Certification be issued to authorize the proposed
impacts There are no additional wetland or stream impacts proposed at this time Please feel
