Part A - Residuals Application Summary: 2010 336-957-8909 Total number of application fields in thepermit: 8 Total number of fields utilized for land application during the year: 6 Total amount of dry tons applied during the year for all application sites: 106.98 Total number of acres utilizes for land application during the year-F 89.4 Part B - Annual Compiance Statement: Facility was compliant during calendar year 2010 with all conditions of the land application permit (including but not limited to items 1-13 below) issued by the Division of Water Quality. e Yes ❑ No If no please, provide a written description why the facility was not compliant, the dates, and explain corrective action to ken. I ) Only residuals approved for this permit were applied to the permitted sites. 2) Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at least 6.0 or the limit specified in the permit. 3) Annual soils analysis were performed on each site receiving residuals during the past calendar year and three (3) copies of laboratory results are attached. 4) Annual TCLP analysis (if required) was performed and three (3) copies of certified laboratory results are attached. 5) All other monitoring was performed in accordance with the permit and reported during the year as required and three (3) copies of certified laboratory results are attached. 6) The facility did not exceed any of the Pollutant Concentration Limits in 15A NCAC 02T .1105(a) or the Pollutant Loading Rates in 15A NCAC 02T .l 105(b) (applicable to 40 CFR Part 503 regulated facilities). 7) All general requirements in as specified in the Land Application Permit were complied with (applicable to 40 CFR Part 503 regulated facilities). 8) All monitoring and reporting requirements in 15A NCAC 02T .I I I I were complied with (applicable to 40 CFR Part 503 regulated facilities). 9) All operations and maintenance requirements in the permit were complied with or, in the case of a deviation, prior authorization was received from the Division of Water Quality. 10) No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of violations are attached to include appropriate actions and remediations. I contacted A&L Eastern Laboratories, Inc. concerning the missing parameters; I found out the following information: 1. The parameters missing had not been analyzed. 2. Too much time has past to have the parameters ran 3. The reason the parameters were not ran was because of confusion on the chain of custody. The confusion on the chain of custody came from box S 1 which is a test for Organic matter, estimated nitrogen release, phosphorus, potassium, magnesium, calcium, pH, buffer pH, hydrogen, CEC, and %.base saturation; box S2 which is Soluble Salts and Sodium, and box S3 which is -test S1 plus sulfur, boron, zinc, manganese, copper, and iron and box S23 which is test S3 plus soluble Salts and Sodium. When the chain of custody was filled out, box S2 was checked instead the box right beside it, box 523; which should have been the one marked. In a discussion with A&L Labs, we have now made it standard practice to automatically run test S23 for all Soil Samples sent in for analysis from Southern Soil Builders, Inc. We regret that these three parameters were missing from the 2010 annual report, however corrective action has been put into place to make sure that all required parameters will be ran in all future soil analysis. Also, a soil analysis has been taken on each field that the City of Marion land applied on in the calendar year of 2010 with all parameters required ran. These samples are at A&L Laboratories, with copies of the results being sent to the Asheville Regional Office as soon as we receive them, which are expected early this week. Also, the inspection noted that the Pathogen and Vecotr Attraction Reduction (VAR) Form was not properly filled out; an updated copy of the VAR from has been included with this letter. Lastly, an updated map to show the reduced acreage for field K-1; due do a new small vineyard that has a water supply well that has been added to the north east corner of the field, is being completed and will be sent to the Division for review when it is completed.
