This DMR form is only applicable to stormwater discharges from the following areas that have been specifically designated by the Division as subject to the requirements of Part F-1 and brought under NCG080000: (1) oil water separators (2) containment structures at petroleum bulk stations and terminals with a total petroleum storage capacity of less than 1 million gallons (3) other stormwater discharges specifically designated. For stormwater discharges associated with vehicle maintenance areas at categorically captured facilities under NCG080000, please use the standard NCG080000 DMR form. Hence, I am sending this information in the format to submit pursuant to the interim procedures that are in effect until NSRR is ready to use eDMR. Attached is the completed DMR, lab results and field qualitative (visual) inspection reports. Although you are already using eDMR for your other individual NPDES permits, I am advised that you must complete steps one and two of the "Six Step Process" (hlt 2s://deg.nc.gov/about/divisions/energy-mineral-and-land-resources/stormwater/stormwater- pro rg am/npdes-industrial) the state has posted to describe the process of transitioning to eDMR reporting for stormwater discharges During this transitional period, until you must begin using eDMR, the procedure is basically to upload the DMR which is attached, online using the upload reporting system (hgps://edocs.deg.nc.gov/Forms/SW-DMR) and mail one copy of the DMR to the state regional office. The following is a more detailed description of what this entails. The attached DMR must be signed, dated, phone number and email added on the bottom of the second page by the Permittee or delegated authorized individual. 2. The signed DMR must then be uploaded to website above by completing the upload cover sheet you will find at this site and attaching a scanned copy of the DMR. I have attached a partially completed cop of the upload transmittal sheet which you will see when you access this site. I have left the signature information portion blank because I do not know who that is.
