find enclosed the Discharge Monitoring Report (DMR) for the monitoring period of
October 1, 2021 to December 31, 2021. Samples were collected on December 30,

Notes:
The facility has implemented mandatory Tier 1 response actions for the 2021 Q4 benchmark exceedance
with the following exceptions:
Notification of the Division's Regional Office was not made.
Stormwater management inspection was conducted in Q12022, but not within the required
timeframe (i.e., 2 weeks).
The 2021 Q4 sampling results are not representative of the facility's storm water discharge. Sampling
results in other quarters were substantially lower. See below for a summary of past analytical results.
Multiple inspections have been completed and no source of increased TSS concentrations were
observed. The site will continue to investigate and monitor discharges.
