Forward one copy of the report to DEQ at this address Raleigh Regional Office NCDEQ 3800 Barrett Drive Raleigh NC 27609 3. Scan and upload the DMR at this web site https:Hedocs.deg.nc.gov/Forms/SW-DMR Look for the upload button 4. Insert the second DMR (three ring punched) copy into Appendix C of your Stormwater Pollution Prevention Plan. The general permit your facility operates under has been updated. One significant change is a requirement to sample four times annually. The sampling windows are as follows-, Jan 1- March 31, Apr 1- June 30, July 1 — Sept 30 and Oct 1- Dec 31. Also note, currently NCDEQ says they are behind in registering users of the EDMR system so there may be some delays encountered.
